| Objective: To evaluate the clinical applications of transvaginalsonography (TVS) and diagnostic hysteroscopy in diagnosis of uterineabnormalities in infertile women.Methods: Retrospective analysed324infertility patients in ourhospital from March2011to December2013.All patients were performedtransvaginal sonography and hysteroscopy.Compared with the results ofTVS and hysteroscopy. Area under the ROC curve(AUC),positivelikelihood ratios (LR+),negative likelihood ratios (LR-),Sensitivity,specificity, positive predictive value (PPV) and negative predictive value(NPV) were calculated for TVS. Hysteroscopy served as the gold standard.Results: The overall AUC, LR+, LR-,sensitivity, specificity, positiveand negative predictive values for TVS in the diagnosis of uterineabnormality was0.811,2.87,0.07,95.5%,66.7%,98.7%and36.4%,respectively. The AUC, LR+, LR-, specificity, sensitivity,PPV and NPV ofTVS in detection of adhesions were0.880,10.38,0.17,84.1%,91.9%,86.9%and90.0%, respectively. These indices were0.821,7.23,0.28,74.5%, 89.7%,78.8%and87.3%for chronic endometritis,0.893,17.35,0.18,83.3%,95.2%,83.3%and95.2%respectively for polyp.Conclusion:TVS can understand the morphology of the uterus anduterine cavity abnormalities, it is a safe,non-invasive,effective,simple,repeatable method which may be used as the initial diagnostic procedure toselect patients for hysteroscopy.And hysteroscopy has confirmed thesignificant value of the diagnosis of chronic endometritis. |
