Objective:To explore the prophylactic effect of different periods useof gabexate mesilate on post ERCP pancreatitis(PEP) and hyperamylasemia(PEHA).Methods: According to different periods use of gabexate mesilate,381patients were divided into preoperative group, postoperative group,complete group and control group. Every treated group was injectedintravenously gabexate mesilate600mg qd, the control group intravenouslyinjection of normal saline. Blood was drawn in each group24h followingERCP for amylase measurement. Clinical manifestations of acutepancreatitis were also observed.Results: At the24th hour after ERCP, serum amylase of every treatedgroup was significantly lower than that of control group(P<0.05).Theincidence rates of PEHA in preoperative group, postoperative group,complete group were28.41%,26.80%,15.84%,which were significantly lower than that in control group46.32%(P<0.05).While, the incidence rateof PEHA in complete group was also significantly lower than that inpreoperative group and postoperative group(P<0.05). The incidence rates ofPEP in preoperative group, postoperative group, complete group were3.41%,3.09%,1.98%,which were significantly lower than that in controlgroup9.47%(P<0.05).Conclusion: Gabexate mesilate can effectively reduce serum amylaselevel and the incidence rate of PEHA and PEP at the24th hour after ERCP.The best way of Gabexate mesilate is that preoperative intravenousinfusion of600mg in6h and600mg after ERCP in12h.
