Objective:To discuss the effect of mean platelet volume(MPV)/platelet count (PLT)(MPV/PLT) and white blood cell count(WBC) on the hospital mortality in S T segment elevation myocardial infarction(STEMI) patients with advanced revascula rization and post PCI operation. Methods:From November2009to August2013i n our hospital,660cases of STEMI patients were included,among which88were d ead.The blood routine,biochemical and cardiac ultrasound and clinical data were col lected.ROC curve was used for the mortality prediction,the admission MPV/PLT an d WBC were evaluated to determine the best clinical cut-off point for hospital mor tality.The logistic regression analysis was used to determine the confounding factors of postoperative mortality of late revascularization of STEMI patients with PCI.Re sults:MPV/PLT ratio in patients died and survived in hospital were(0.064505±0.046226)11/(109/L)、(0.050296±0.022412)tl/(109/L) respectively, there was statistically diff erence(P<0.001).WBC in patients died and survived in hospital were(12.83±5.55)×109/L、(9.16±3.44)×109/L respectively, there was statistically difference(P<0.001).The ROC curves were used to determine the clinical cut-off point、the cut-off point for MPV/PLT was0.047460fl/(109/L), the cut-off point for WBC was9.28×109/L.MPV/PLT ratios above and below0.047460fl/(109/L) had late revascularization proportio n mortality of19.2%and8.1%after PCI.there was the statistical significance(P<0. 001).WBC above and below9.28×109/L had late revascularization proportion mortal ity of22.6%and6.4%after PCI,there was the statistical significance (P<0.001). M PV/PLT(OR0.524,95%CI:0.292~0.940,P=0.017) and WBC(OR0.199,95%CI:0.109~0.363,P<0.001) were independent risk factors for late revascularization morta lity after PCI in the STEMI patients in hospital. Conclusions:MPV/PLT and WBC were valuable predictive factors for late revascularization mortality after PCI in the STEMI patients in hospital. MPV/PLT>0.047460fl/(109/L)and WBC>9.28×109/L w ere independent risk factors for late revascularization mortality after PCI in the ST EMI patients in hospital.
