Objective:To compare the therapeutic result of EMR and ESD for early esophageal cancer and precancerous lesions which diameter between2cm and3cm. Methods: Retrospective analysis of58patients with early esophageal cancer and precancerous lesions who had accepted endoscopic treatment in our hospital from January2008to January2013.And the diameter of lesions in both groups were between2cm and3cm. There were31patients with37lesions accepted EMR, other27patients with34lesions accepted ESD. To analyze the effectives of two endoscopic treatment methods, by observing the en bloc rate, blood loss, perforation, the incidence of postoperative and esophageal stricture recurrence. Results:The en bloc rate in EMR group is56.8%(21/37) and in ESD group is88.2%(30/34), P<0.05, and the differences between them have statistical significant. Bleeding rate in EMR group is2.7%(1/37) and in ESD group is17.6%(6/34),.P<0.05, and the differences between them have statistical significant. Perforation rate in EMR group is0(0/37) and in ESD group is11.8%(4/34), P<0.05, and it is different in statistics. The EMR group stenosis rate is10.8%(4/37) and in ESD group is8.8%(3/34), P>0.05, two groups of postoperative restenosis rate have no significant difference. The local recurrence rate of EMR group is13.5%(5/37) and in ESD group is0(0/34), P<0.05, and it is significantly different. Conclusion:The effect of ESD for treating early esophageal cancer and precancerous lesions which diameter of lesions are between2cm and3cm is much better than the EMR group. But ESD has the risk of bleeding and perforation. Both of them in the postoperative esophageal stricture have no significant difference.
