| Diabetes mellitus is a group of metabolic diseases characterized by hyperglycemia. Periodontitis was designated as the sixth complication of diabetes. They interact on each other in inflammation course. In clinical,the patients with diabetes and periodontitis are more serious destruction in periodontal tissue,and more chanllenge in therapeutic method.Coenzyme Q10(Co Q10) is also known as ubiquinone because of its ubiquitous presence in nature and its quinone structure(similar to that of lipovitamin). Human and mammalian side chain contains 10 coenzyme Q different pentene units, so called the coenzyme Q10.Coenzyme Q10 plays four major roles:transporting electrons;anti-oxidant;reduce apoptosis and anti-inflammatory.It has been proved a good effect in some disease,such as cardiovascular diseases,diabetes mellitus,tumor and so on. So it would also be a good effect that Co Q10 were applicated in patients with diabetes and periodontitis.Objective:To establish the periodontitis rats model with type â…¡ diabetes,and administrated with coenzyme Q10, then explor the effect of Co Q10 to the periodontal tissue with diabetes periodontitis by monitoring the damage of periodontitis, detecting the IL-17 in ginginval tissue,and measuring the level of RANKL and OPG in serum.Trying to provide a theoretical basis for clinical drug prevention and treatment of diabetes periodontitis.Methods:1 Experimental animals:96 male Wistar rats, weighing approximately 180-220 g were divided to 6 groups randomly,each group contained 16 rats.They are normal control group(NG),periodontitis group(PG);diabetes mellitus group(DG);diabetes mellitus with Co Q10 group(DCo G);periodontitis and diabetes mellitus with physiological saline group(PDNa G)and periodontitis and diabetes mellitus with Co Q10 group(PDCo G).Every group was treated with the corresponding measure.Established the periodontitis model with silk ligatured around the dental cervix,and established the diabetes mellitus model with injection of streptozocin(STZ).2 After the successfully setting up the animal model, at the beginning of the 7th week(adaptive feeding is the first week), DCo G and PDCo G were fed daily 10% water-soluble coenzyme Q10 9mg/kg, PDCo G were fed the same dose of normal saline, both for eight weeks.3 The weight, mean depth of periodontal pocket and total attachment loss of rats in each group were measured and analyzed statistically before administered and 7th, 9th, 11 th, 15 th week after administered. Rats in each group were controlled with appropriate anesthesia. Open the chest and take cardiac blood about 5 ml into the routine blood-collecting vessels. After being setted aside 30 mintues take 1ml serum to Eppendorf;Then rapidly taken the periodontal tissues of mandible of mandibular first permanent molar of rats and placed 4% paraformaldehyde. The gum tissue were dehydrated, embedded in paraffin, done paraffin sections, HE and immunohistochemical stained after antigen retrieval, observed by microscope, and Enzyme-Linked Immuno Sorbent Assay(ELISA) detect RANKL/OPG levels.Results:1 Establishment of periodontitis with typeâ…¡ diabetes mellitus animal modelBefore the injection of STZ, blood sugar had no significant differences in groups.The 6th week(1 week after injection STZ) the blood glucose were more than 16.7 mmol/L,and there was a decrease in weight in DKG, DCo G, PDNa G and PDCo G. The rats were made diabetes mellitus model successfully. Blood sugar were less than 16.7 mmol/L in KG and PKG. PKG, PDNa G and PDCo G all reached the standard of periodontitis: periodontal probing depth increased(PD>0.4mm), the vertical height of alveolar bone decreased;Compared with KG, DKG and DCo G, the periodontal pocket depth7 and the vertical absorption height of alveolar bone were statistically significant(P<0.05) in PKG, PDNa G and PDCo G, namely the periodontitis building success. 2 the pathologic changes of the local gum tissue 2.1 HE stainingAt 7th week, there were slight inflammatory cells in gingival epithelium and lamina propria of DCo G, it were also in PDNa G and PDCo G, and the gum collagen fibers disordered arrangement. As the time gone, after the 15 th weeks, the tendency of inflammatory cells in the epithelium and lamina propria decreased in DCo G PDCo G, and periodontal collagen fiber hyperplasia and more regulated than PDNa G. 2.2 Sirius scarlet dyeingAs the time gone, there was no significantly change in DCo G,The collagen type I and type â…¢ both increased, but typeâ…¢ increased in proportion in DPCo G. 2.3 immunohistochemical detectionIL-17 was expressed in each group. Comparison between groups: the 7th week, it is the least expression in NG, DG, and DCo G, there were no difference between groups, but significant difference with other groups(P<0.05);the expression increased in PG, and there were statistically significant differences compared with the other groups(P<0.05); it is the the highest expression in PDCo G and PDNa G, there were no difference between them, but statistical difference compared with other groups(P<0.05). At the 9th and 11 th week, the comparision result between each group is the same as the 7th week; at 15 th week,the expression were no difference in NG, DG and DCo G, but statistical difference compared with other groups(P<0.05); the expression is the least in PDCo G, and there was statistical difference compared with other groups(P<0.05).Comparision between moment in Each groups: the expression of IL-17 has no statistical difference in each moment among NG, PG, DG and DCo G; In PDNa G the expression was the most at 15 th week, and there wasstatistically significant difference compared with 7th,9th and 11th week(P<0.05);in PDCo G the expression decreased with times gone,compared the 7th and 9th week,there was no difference between the two groups,but there was a significant difference compared with other moments(P<0.05),it is the same as the comparision between 11th and 15th week.3The levels of RANKL/OPG in serum 3.1 Comparison between groups:the level of OPG and RANKL were no difference at the same moment between NG and PG,but there was a significant difference compared with other groups(P<0.05),and it is also between DG,DCo G,PDNa G and PDCo G.The comparison of the RANKL/OPG ratio between groups: The ratio is higher in NG and PG at each moment, there were no difference between groups, but were significant difference compared with other groups(P<0.05).At 7th week, the ratio is low in DG, DCo G, PDNa G and PDCo G, and no difference between groups. At 9th week, the ratio is lowest in PDNa G, and have significant difference compared with other groups(P<0.05);at 11 th week, there were no difference in DCo G, PDCo G and DG,but had significant differences compared with PDNa G(P<0.05).At 15 th week there were no difference between PDCo G and DCo G,but have significant difference compared with other groups(P<0.05);PDNa G have significant difference compared with other groups(P<0.05). 3.2Comparison between two moments in each groupThe level of OPG and RANKL were no statistical differences in each moment among groups.Comparison of RANKL/OPG ratio between group: There were no statistical difference in NG, PG, DG and PDNa G all the time;DCo G set a smaller ratio at 7th and 9th week, there were no statistical difference between the two groups,but were significant difference compared with ratio of 11 th and 15 th week(P<0.05);the comparision between 11 th and 15 th week was also in PDCo G,but at 15 th week, the ratio increased,and have significant difference compared with other moments(P <0.05).Conclusion 1 The experiment successfully copied the rat periodontitis with type â…¡ diabetes model through the silk ligation and fat-sugar enriched diet accompanied by STZ intraperitoneal injection.2 The experiment results indicate that administration with a certain dose of Co Q10 could improve systemic and periodontal local indexes of rats, slowing attachment loss and alveolar bone resorption; and improve periodontal tissue symptom in the diabetic periodontitis rats;improve the proportion of collagen content. So it is helpful to the recovery of inflammation. 3 The result in serum showed that administration with a certain dose of Co Q10 could make the ratio of RANKL/OPG in serum of typeâ…¡ diabetic periodontitis rats close to blood sugar normal rats. 4 The study suggests administration with a certain dose of Co Q10 in clinical is helpful in the patients with typeâ…¡ diabetes periodontitis. |
