Objective to study the curative effect of the vacuum sealing drainage,(the VSD)device applied on the large complex wound treatment by different negative pressure difference.method Selection a total of 96 cases from February 2013 to June 2014 were treated in large complex wound,including 48 cases of upper limb wound and 48 cases of lower limb wound,according to the wound site, and admission time sequence patients were randomly divided into eight groups,including four groups of upper limb wounds and four groups of extremities especially wound, 12 cases in each group.respectively,, all adopt the closed negative pressure drainage treatment after surgical debridement, apply different negative pressure treatment, observation of contrast between groups wound granulation tissue growth, tube plugging rate and the amount of bacteria.Results Eight groups patients in the observation the finish all visible growth of granulation tissue,(1) the upper extremity wound group: 50-200 MMHG(1) set of granulation tissue is rich, including 10 patients with postoperative week meet immediate free skin graft or skin flap transfer operation conditions, no infection, fewer complications, blocking pipe(3 cases); 200-350 MMHG granulation tissue(group 2) is the most abundant, including 10 cases of postoperative week conforms to the operation condition, plugging pipe in 2 cases, 350-500 MMHG(group 3) granulation tissue is also rich, 8 cases of postoperative week conforms to the operation condition, plugging pipe in 2 cases; 500-650 MMHG(group 4) granulation tissue, 7 cases of postoperative week conforms to the operation condition, plugging pipe 0 cases;(2) the lower limb parts wound group: 50-200 MMHG(group 5) growth of granulation tissue is abundant, 8cases of postoperative week conforms to the operation condition, plugging pipe(3cases); 200-350 MMHG rich group(6) granulation tissue growth, 10 cases of postoperative week in accordance with operation conditions, blocking pipe in 2 cases;350-500 MMHG(group 7) granulation tissue growth best, 10 cases of postoperative week conforms to the operation condition, plugging pipe 0 cases; 500-650 MMHG group(8) granulation tissue growth, 7 cases of postoperative week conforms to the operation condition, plugging pipe 0 cases. Quantitative bacteria: group 1 > 3 > 2 >group 4 and group 5 > 6 > > 7 group 8, but in group 2 and group 4, 5 and 7 and 8, group both comparison there was no significant difference between groups of 6.(P > 0.05).Conclusion The effect of treatment on the upper part by the wound VSD technology in200-350 mm Hg is better, however, the best application of negative pressure in350-500 mm Hg is used in the lower extremities wounds.
