Clinical Research Of Dental Implant Therapy For Patients With Severe Periodontitis

Objective To observe the problems and short-term outcome of clinical implant therapy in 42 patients with severe periodontitis.And to provide clinical guidance for developing implant treatment in patients with periodontitis.Methods A total of 42 partially edentulous patients due to severe periodontitis were choosen for the present study. All patients accepted systematic periodontal examination,diagnosis and treatment,actively carry out oral hygiene education,and take surgery of extraction site preservation if necessary.All of the patients accept implant treatment after systematic periodontal treatment only when PLI<20%,sites of PD≥5mm not exceed 5%,percentage of BOP(+)<25%.Use CBCT to evaluate the bone tissue in the implant area before the surgery,and use the technology of maxillary sinus floor elevation or guided bone regeneration or bone extrusion to solve the problems of bone defect or bone density defect during the surgery.A total of 80 implants implanted and implant supported dentures were made 3~6 months after the implant surgeries according to every patient’s situation.Peri-implant tissue conditions were monitored immediately after the denture restorations and 1,3 months later, the indexes are PD,m SBI and m PLI of implants.Result A total of 80 implants placed, all implants were placed passively with good initial stability.Of which 6 sites for extraction site preservation,12 sites for maxillary sinus elevation,and 31 sites for GBR. During the observation period,all the implants were retained,no loosening,no percussion discomfort,X-ray showed that the bonearound the implant without exception.All implants were in good function and no significant inflammation was found in peri-implant surroundings.Compared the PD,m SBI and m PLI of the implants after 1 months and 3 months with which immediately after the restoration,the difference were not statistically significant(P> 0.05).Conclusion Through strict infection control and formulating normative treatment plan,patients with severe periodontitis can obtain satisfactory short-term clinical outcomes of implant therapy.Comprehensive periodontal treatment and dealing with bone defects are the key influencing factors.
