| Objective:To evaluate the feasibility and safety of laparoscopic liver resection and introduce the surgical technique.Materials and Methods:A retrospective study was carried out on 137 patients with hepatocellular carcinoma (HCC) who met the inclusion criteria for laparoscopic liver resection(LLR) were treated from January,2010 to December,2014 in department of hepato-biliary-pancreatic surgery, Qilu Hospital of Shandong University.. They were performed by laparoscopic and open hepatectomy on a voluntary basis. We collected the patients clinical features such as gender, age,positive incidence of serum Hepatitis B surface antigen (HBsAg), suffered from hepatic sclerosis or not, serum levels of alpha fetoprotein (AFP), liver function before and 7 days after operation, operation time, length of incision, volumes of infused blood and loss of blood, size of liver lesions, pathological type, practical first eating time after operation, drainage tube removal time, hospitalization time, complication rate for statistical and study analysis.Results:All operations were successfully performed. No operative deaths occurred in either group. There was no conversion to open surgery in the Laparoscopic hepatectomy group. Two groups’ general information have no significant difference(p>0.05).Laparoscopic hepatectomy group is significant(P<0.05) less in length of incision, operation time, first eating time after operation, drainage tube removal time, and hospitalization time than the open operation group. No significant difference (p>0.05)was found in liver function after operation, volumes of blood loss and transfusion during the operation and the incidence of complications between the two groups.Conclusion:Laparoscopic liver resection is a safe surgical approach for treating malignant hepatic lesions, with an equal therapeutic effect to that of traditional open hepatectomy. It should be performed by a surgeon with sufficient experience in open hepatic resection and who is proficient in laparoscopy. Laparoscopic hepatectomy is characterized by excellent visualization, minimal trauma, rapid postoperative recovery, short hospital stay period, significantly improved the quality of life of patients after surgery. It is worth of promoting the application clinically. |
