The Early Result Of Using Jumbo Cups For Acetabular Deficiencies In The Total Hip Arthroplasty Revision

Objective:To study the recent clinical and radiographic curative effect of using Jumbo cups in repair of acetabular bone loss or defects in the total hip arthroplasty revison.Methods:The retrospective study included 23 cases(23 hips)for acetabular deficiency after total hip arthroplasty(THA) from April 2010 to Decembr 2014. There were 11 males and 12 females, whose average age was 59.5 years old (range:54-68 years). Primary revision was performed on 22 patients, while the other one was secondary revision. As to the reason of acetabular revision, there were 22 cases caused by aseptic loosening,1 case by infection after total hip replacement. According to the Paprosky acetabular defect classification, there were 4 hips of type Ⅱ A,6 hips of type Ⅱ B,5 hips of type Ⅱ C,4 hips of type Ⅲ A and 4 hips of type ⅢB. Among them 5 cases were repaired with screw cup(63mm～72mm), and 18 cases with TM acetabular cups (52mm～68mm), combining with bone graft or fixed by screw. The Harris hip score were used to evaluate the hip function and state. The prosthetic stability and bone union were judged based on the X-ray.Results:All the patients were followed up for a mean period of 21.78 months (range:4-57 months).The Harris hip scores were (37.39±6.77) points (25-47points) before operation, and (84.04±4.07) points (77～91points) after the operation (P<0.01). From the X-ray, we can partly found continious trabecular bridging between the bone graft and the host bone,and no cup lossening occurred. During the follow-up visit,the pain was disappeared or disappeared basically,and no complication such as prosthetic dislocation、infection、rejection、nerve damage appeared.Conclusions:The early result of Jumbo cups for acetabular defect was good. The technique is simple and effective.
