OBJECTIVE:The aim of the present study was to evaluate the impact of standardization, homogenization pharmaceutical care,led by clinical pharmacists for patients with COPD,on medication adherence,the rational use of antibiotics,the incidence of adverse reactions,patient satisfaction,referral,the readmission rate and medication errors.METHODS:In the study we designed a prospective,randomized,controlled trial,and provided standardization,homogenization pharmacy services for COPD patients hospitalized. Patients met the inclusion criteria will be randomly assigned to the experimental group and the control group. Test group patients received clinical pharmacist-led pharmacy service during the whole process of hospitalization,while the control group received only the traditional medical services; And all patients were followed up one month after discharge. The main indicators were medication adherence and rational use of antimicrobial agents (including usage,combination rate,intensity of use), and the secondary inspection indicators were adverse drug events,patient satisfaction, referral or readmission after discharge and medication error rates.RESULTS:The results showed that when compared with the control group,the medication adherence and the rational use of antimicrobial agents of patients in test group were significantly higher (P<0.05). The satisfaction of traditional medical procedures and medication monitoring process among patients in test group increased significantly (P<0.05). Referral or readmission after discharge and medication error rate among two groups had no significant difference (P>0.05).In the test group,the incidence of adverse drug events of the test group is significantly reduced (P<0.05);CONCLUSION:The standardization and homogenization of the pharmaceutical care led by clinical pharmacist for patients with COPD accept multi-drug therapy in hospital, can significantly improve medication compliance,satisfaction and the rational use of antimicrobial agents. Significantly reduce the incidence of adverse drug events was examined in the test group. But the referral or readmission after discharge and medication error rate among two groups had no significant difference. It is important that clinical pharmacists conducted standardized pharmacy services to improve clinical and humanistic outcomes in COPD patients based on this study.
