Lean Management Application In Configuration Link Of Pharmacy Intravenous Admixture Service

Objectives: The pharmacy intravenous admixture service(abbreviated as PIVAS)has been experiencing new reform and developmentin in China nowadays.PIVAS can offer timely,efficient,safely and rational intravenous medication to patients.Compared with Ward configuration,PIVAS has more complicated aspects of the work, the circulation process and more particularly centralized configuration,distribution and quality control and other aspects of the complex,easy to cause the higher error rate,because drug configuration errors can cause serious bodily harm and economic loss.So,how to avoid or reduce errors,especially Reduce drug configuration error,has attracted the attention of many experts and scholars.Lean management is an important means of carrying out the link control,promoting the quality control,it can direct the conventional management to further.The essence of lean management coincide with the requirement of PIVAS configuration accurately and on time,It is useful for the establishment of scientific and objective of quality management system.The object will be lean management concept,connotation,method introduced to the managemen of PIVAS,to achieve the quality strives for perfection,accurate configuration on time,reasonable division of labor refinement,Operating strictly regulate purpose.Methods:In this study adopts literature review,empirical analysis,comparison before and after,the methods of lean management methods and techniques.Literature research:This study systematically check the literature and data related lean management and PIVAS quality management,data mainly comes from CHKD periodicals full text database,Wanfang medical network,Dayi medical network and Chinese journals database.By collecting the theoretical and practice of lean management,analys Lean management related theory and the effect of the specific application in hospital management.Through the collection of the common problems and improvement measures in PIVAS of research status,the further understanding of the quality management methods.PIVAS empirical analysis:in this study,a hospital PIVAS as a pilot,Through on-the-spot tracking,understand the configuration status,prepare the way for constructing PIVAS lean management system.1 Summary analysis the effect factors of PIVAS configuration speed and configuration quality using the method of fishbone diagram,to find out the reason affected configuration speed and configuration secutity.2 Deal with the problems,the use of 5S management,,visual management, value stream mapping, standardization, motivation and leading staff and other lean management methods and tools,Through perfecting the rules and regulations and job responsibilities,detailed division of responsibilitiest, standardized the operation process,optimized the information system and other means,to reach people-canonical, business-processes,objects- standardization, clear people and things,and the division of responsibility for the purpose.With minimal staff,time,equipment and space to create the value of as much as possible.Before and after comparison:Comparison and analysis the survey data before and after improvement,to evaluation the improved effect.Evaluation the validity of the lean management for improve the PIVAS configuration speed,research on lean management theory whether can be successfully applied in the PIVAS configuration of quality management.Results:1 Statistical analysis of the problems of PIVAS configuration link, Correct drugs is low,Personnel division of labor is not reasonable,workflow is imperfect,Configuration tasks and the order is not detailed,Operation is not standard are five aspects,the main reason to reduce allocative efficiency lower and the safety of the configuration.2 Adopt the method of lean management to improvement the work of PIVAS,the following results after implementation:â‘ 01 group of common drugs configuration speed has improved significantly,by the t test,P<0.01;01 group of antibiotic drugs configuration speed has improved significantly,by the t test,P<0.05; â‘¡The situation of Configuration errors using Possion distribution,Comparing the two sample mean,Drugs do not match, he solvent label wrong,Configuration error,finished infusion visible stopper,liquid leakage Is were significantly decreased,P<0.05,It has statistical significance.Conclusion:1 PIVAS in different operating mode,Different personnel,Different personnel division of labor,Lead to the problems caused by the type and severity of will also be different.Focus on PIVAS appear all sorts of problems,Analysis of the root causes of problems,is to carry out the lean management, improve the quality and speed of configuration necessary.2 By means of PIVAS work field trip,a comprehensive understanding of the configuration link quality management status,the lean management combined with PIVAS quality management,using the lean management method to improve the existing problems,can achieve significantly improve the efficiency of configuration,reduce the hidden dangers and errors occurred. Further confirmed the lean management is feasible for quality management applied in PIVAS configuration link.
