Observing the effect of external application of "YIDANTIE" treating severe acute pancreatitis(SAP) rats on serum motilin, vasoactive intestinal peptide and gastric emptying rate,intestinal impelling rate, and then discussing the pathogenesis of gastrointestinal motility disorder in SAP rats and the mechanism of "YIDANTIE" acts on SAP rats` gastrointestinal motility disorder, to provide experimental Foundation and the theoretical basis for “YIDANTIE†treating severe acute pancreatitis in clinic.Methods : Methods: Divided 45 male healthy SD rats into 3 groups by random allocation, group J: sham-operation group, group M: SAP model control group, group Z: “YIDANTIE†treat group. SAP model was induced by retrograde injection of 4% sodium taurocholate(ST, 1.0ml/100g) in biliopancreatic duct with speed of 0.1ml/min in M group and Z group. Group J were operated the same methods of group M and Z without pumping sodium taurocholate, respectively, keeping the transfusion needle in biliopancreatic duct 5min and then closing abdomen. All rats were injected saline solution 2ml/100 g for supplement of humoral losing. After operation,Group Z were external-applied “YIDANTHEâ€on midsection while group J and M were external-applied sterile gauze. All rats keep warm in a room of 25℃, prevent of asphyxia by glossocoma and vomit, ambrosia and free of drinking. All rats werechanged the dressing on midsection per 24 hours, and after 48 hours of surgery, all rats were perfused 2% blue dextran(0.4ml) into stomach. After 30 minutes of intragastric administration, open the abdomen to observe the general condition of pancreas, intestines and ascites, and detect amylase, blood calcium, motilin, and vasoactive intestinal peptide(VIP); measure the damage score of pancreas by pathological section; count gastric emptying rate(GER) and Intestinal propulsive rate.Results: â‘ .General condition of enterocoelia: The pancreas of sham-operation group(group J) appeared pale yellow, calcifications, hemorrhage and necrosis can not be found, flatulence of stomach and intestines also can not be found. The model control group(group M) can observed large amount of calcifications and red exudant, congestion, edema and flaky necrosis can be seen on pancreas, adhesion around pancreas appeared in some rats, flatulence of stomach and intestines can be seen. Similar appearance of group M can be observed in “YIDANTHE†treat group(group Z) but decreased symptom than group M. â‘¡.Serum amylase: Remarkable high concentration of serum amylase group M and Z compared with group J, which difference has statistical significance(P<0.01); While group Z has lower concentration than group M, the difference has no statistical significance(P>0.05). â‘¢.Serum Ca2+: Compared group M and Z with group J, the concentration of Ca2+ in serum was much lower, which has statistical significance(P<0.01), while group Z was higher than group M, but which did not has statistical significance(P>0.05).â‘£.Serum MTL: Much lower level of serum MTL can be seen compared group M/Z with group J, statistical significance was significant(P<0.01); Group Z has higher serum MTL than group M, which had statistical significance t(P<0.05).⑤Serum VIP: Much higher level of serum VIP can be seen compared group M/Z with group J, statistical significance was significant(P<0.01); Group Z has lower serum VIP than group M, which had statistical significance(P<0.05).â‘¥gastric emptying rate: gastric emptying rate: group M has lower rate than group J, the difference has statistical significance Remarkable(P<0.01);group Z has higher rate than group M, the difference has no statistical significance(P>0.05). ⑦intestinal impelling rate: Compared group M and Z with group J, the rate of intestinal impelling was much lower, which has statistical significance(P<0.01), group Z was higher than group M, which did has statistical significance(P<0.05).â‘§The pancreas pathological grading: group M and Z have remarkable lower grading than group J, which difference has statistical significance(P<0.01); group Z has lower grading than group M, the difference has no statistical significance(P>0.05). ⑨ the concentration of Serum motilin was positively correlated to intestinal impelling rate(r=0.798;P<0.01);the concentration of Serum VIP was negatively correlated to intestinal impelling rate(r=-0.664;P<0.01)。Conclusion: 1. the gastric emptying rate and intestinal impelling rate of group M has lower rate than group J,It indicates that there is gastrointestinal motility disorder in SAP rats.2. the concentration of Serum motilin waspositively correlated to intestinal impelling rate;the concentration of Serum VIP was negatively correlated to intestinal impelling rate;Group M has higher serum VIP than group J,and group J has higher serum MTL than group M,It indicates that gastrointestinal hormones disorder could be one of the reasons of gastrointestinal motility disorder in SAP rats.3. The rate of intestinal impelling in group Z was higher than group M, It indicates that “YIDANTIE†could improve the gastrointestinal motility in SAP rat,and Its mechanism may be related to the secretion of VIP and MTL. |
