Objective:To explore the feasibility and effect of facial skin lesions reconstruction with the expanded scalp flaps and hair removal with laser. Methods:Between January 2010 and December 2014,14 cases with facial skin lesions were treated with expanded scalp flaps transplantation and hair removal with laser in department of plastic and aesthetic, the first affiliated hospital of Guangxi Medical University. The whole treatment process could be subdivided into three stages. At the first stage, the soft tissue expander was inserted into the donor site, periodic water injection postoperatively. During the second stage, the facial lesions were excised, and the soft tissue expanders were removed. Design the expanded scalp flaps according to the shape and scope of the facial skin defects. Forming the island or advanced skin flaps, and then transferred into recipient site to cover the facial skin defects. When the wound healed completely, began to removed the hair of scalp flaps with laser, a total of 4 to 6 times, interval time 1 month. Results:At the first stage, the size of placed expanders ranging between 160 ml and 500 ml, expanded for 10 to 24 weeks, with an average of 14.8 weeks. In addition to one case of facial expanders occurred leakage, the rest no complications occurred. In the second stage, the minimum flap area was 14.0cm X 5.0cm, the maximum flap area was 16.0cm× 14.0cm.After transplantation, island flap is about 3 to 5 days of swollen. Eventually, all of flaps were survived completely,1 flap occurred distal partial flap necrosis. All of donor site sutured directly, and scars were concealment. The third phase of hair removal with laser obtained good effect after more than 6 months of follow-up. The color, texture and thickness of scalp flaps matched well with the facial region. Conclusions:This research verified the feasibility of facial skin lesions reconstruction with the expanded scalp flaps and hair removal with laser, its effect was satisfied. It was a good option to reconstruct the larger facial skin lesions.
