| Objective: In order to understand egress and medical examination condition of pregnant women and 0 to 3-year-old children in a Country in Anhui Province. Study the influence of egress to pregnant women’s and children’s system management rate. Method: Ambispective cohort study was adopted to study pregnant women. Self-designed questionnaire was used to gather egress and health care services accepting information of pregnant women. Maternal checkups information were collected according to the maternal health care service schedule. For those pregnant women who failed to have a medical examination timely should take a questionnaire to know the orientation of egress and situation of accepting a physical examination. End-point was 60 days after postpartum. Prospective cohort study was adopted to study 1 to 3-year-old children. Children physical examination situation were collected according to the child service schedule. For those children aged 1 to 3 that failed to have a medical examination timely should take a questionnaire to know direction of egress and situation of accepting a physical examination. End-pointwas 3-year-old. Terminal time of observation was September 30, 2013.Data were entered and analyzed by Epidata and SPSS 16.0. Result: In pregnant and lying-in women cohort, 6281 person-time were checked,4819 person-time accepted physical examination timely,1233 person-time doing the questionnaire, number of validated questionnaire was 1143, 229 person-time losing to follow-up. In children aged 0 to 1 cohort, 5838 person-time were checked,4048 person-time accepted physical examination timely,1345 person-time doing the questionnaire, 445 person-time losing to follow-up. In children aged 1 to 3 cohort, 4128 person-time were checked,2663 person-time accepted physical examination timely,994 person-time doing the questionnaire, 471 person-time losing to follow-up. The proportion of egress of pregnant women and children in this county was 30.1% and 21.7%.The primary destination of egress was developed area with more employment opportunities such as Shanghai, Zhejiang, Jiangsu and other countries in Anhui province. In the context of follow-up objects and losing to follow-up objects has received the same time of physical examination, the flow-out pregnant women’s system management rate was lower than pregnant women’s at home. The physical examination rate of children at home of each moon’s age was higher than go out children’s, but the system management rate was the same. The possible reason was the high rate of losing to follow-up of children at home. The impact factor to pregnant women was about18.52%~23.63%,to children aged 1 to 3 was about 17.68%~22.98%. Conclusion: The proportion of egress of pregnant women and children in this county was 30.1% and 21.7%. The primary destination of egress was developed area such as Shanghai, Zhejiang, Jiangsu. Pregnant Women: The system management rate of flowing-out pregnant women was 58.5%.The system management rate of annual reported was 50.77%. The actual system management rate of pregnant women in this county was 73.61%(Assuming that lost to follow-up personnel did not receive health care services) and 81.96%(Assuming that lost to follow-up personnel received health care services). The influence of egress on system management rate was 18.52%~26.37%. Children under 3-year-old: The system management rate of flowing-out children was 81.4%.The system management rate of annual reported was 51.07%. The actual system management rate of pregnant women in this county was 68.82%(Assuming that lost to follow-up personnel did not receive health care services) and 86.53%(Assuming that lost to follow-up personnel received health care services). The influence of egress on system management rate was 17.67%~35.37%.In the further work of maternal and child health care work, we should pay attention to collect physical examination information in local area. At the same time, in area of high egress proportion, we should collect health care information of people outside. |
