| Background:Medical adverse events have relatively high incidence rate and cause harm to patients easily. Nursing adverse events are in close relation to quality control and nursing administration and have attracted wide attention. The report of nursing adverse events could prevent the happening of similar events once more and provide evidence for continuous quality improvement as well. Clear reporting category of adverse events, explicit definitions and sufficient cause information of specific events are indispensable parts in reporting process. However, nursing adverse events’ reporting content of hospital in Beijing need to improve in several aspects.Objectives:To develop reporting category, definitions and content of nursing adverse events of hospital in Beijing.Methods:The study was conducted from March 2014 to April 2015. Based on literature review, the category and definition of nursing adverse events were developed by expert meeting. The consultation letter was formed according to the result of expert meeting, literature review and Swiss cheese model. Every nursing adverse event’s reporting content was developed by using the Delphi technique. One hospital in Beijing has used the new formed content as a trial run for one month. Combined with the data of this trial run, the evaluation of the new formed content using condition was explored by semi-structured interview. Totally eight experts attended the meeting, fifteen experts conducted two rounds of consultation letters and five experts took part in the interview.Results:Nursing adverse events that need reported of hospital in Beijing include pressure sore, fall/falling from bed, unplanned extubation, medication error and accident. Reporting content of these events in details has also been obtained. During the one-month trial run,14 nursing adverse events were reported. Experts participated in the interview agreed that the new formed content was better than that at present.Conclusion:The reporting category, definitions and content of nursing adverse events of hospital in Beijing have been developed. According to Swiss Cheese Model, the cause reporting part of every event contains systematic factors, personal factors and environmental factors. The new formed reporting content has been improved, but it still needs further improvement in clinical nursing practice. |
