Clinical Observation Of Xiexin Decoction Plus Jiaoqi Powder Combined With Western Medicine In Treating Acute Nonvariceal Upper Gastrointestinal Bleeding

ObjectiveTo observe the clinical efficacy and safety of Xiexin decoction plus Jiaoqi powder combined with esomeprazole sodium in the treatment of mild to moderate ANVUGIB patients who has stomach-heat syndrome and evaluate the advantages and disadvantages of this treatment comparing with the group treated with single western medicine.MethodAccording to inclusion and exclusion criteria 64 patients were selected and assigned into a test group of 33 patients and a control group of 31 patients by means of random number table. All patients were treated with routine supportive care such as fasting or liquid diet, fluid replacement, plasma expander, correction of electrolyte disorders according to the disease. In addition, the control group was given esomeprazole sodium injection 40mg twice one day; the test group were treated on the base of control group and taked Xiexin decoction(rhubarb charcoal 15g, scutellaria 10g, coptis chinensis 10g, water decoction to 150ml, taking twice a day) plus Jiaoqi powder(a pulvis of colla corii asini powder 6g and radix notoginseng powder 6g, taking twice a day). Both group were treated for five days. After treatment we observed and compared the total efficiency, the mean hemostatic time, TCM syndrome effect, the change of RBC and HGB of the two groups and adverse reactions. Data was collected and analysised by using statistical software called Statall.0.Result1. The clinical efficacy:the total efficiency of the test group(100.00%) was obviously better than the control group(93.33%), the difference was statistically significant(P<0.05).2. TCM syndrome effect:the total efficiency of test group was 100.00%, which control group was 93.33%, the difference was statistically significant (P<0.05). After the treatment, the symptom scores of test group reduced significantly. The difference value between before and after treatment in the test group was more than that in the control group (P<0.05).3. The mean hemostatic time:the mean hemostatic time of the test group(1.81±0.75 days) was shorter than that of the control group(2.40± 1.00 days).The difference was statistically significant (P<0.05).4. Change of RBC and HGB:RBC and HGB of the test group significantly increased after treatment, and the difference has statistics significance (P<0.05). RBC and HGB of the control group did not have significant difference after treatment(P>0.05). The mean RBC and HGB of the test group were both slightly higher than that of the control group, and the difference value between before and after treatment in the test group was more than that in the control group (P<0.05).5. Two groups of patients were tested blood, urine, stool, liver and kidney function before and after treatment, adverse impact was not found on these indicators.ConclusionThe clinical efficacy of Xiexin decoction plus Jiaoqi powder combined with western medicine in treating mild to moderate ANVUGIB was sure. Compared to single western medicine treatment, the combined treatment could significantly shorten the patient’s hemostatic time, improve efficiency and clinical symptoms and increase the level of RBC and HGB. And the security of clinical application is good.
