ObjectiveTo improve the maternal health during the pre-pregnancy and perinatal period and achieve the expected clinical target of promoting maternal and infant health by applying "phsyco-prophylax-is analgesia" comprehensively. Methods538 cases of pregnant women were randomly divided into observation group and control group with 269 cases in each. According to the principle of informed consent, the pregnant women in observation group were received "phsyco- prophylax-is analgesia" clinical intervention measures comprehensi vely: pregnancy systematic training, doula, continued follow-up guidance. The pregnant women in control group were given routine obstetric care, traditional labor and delivery and follow-up. The maternal blood pressure, weight, gestational diseases, mental state, delivery, neonatal Apgar score and weight, the complications of mothers and infants, breastfeeding, puerperal disease, the relationship between doctors and patients, infant illness and development were recorded.The data were analysed by t-test, χ2 test, non parametric test. Results①The incidences of abortion and anemia in observation group were significantly lower than those in control group(p<0.01). The incidences of hypertensive disord complicating pregnancy and diabetes were lower than those in control group(p<0.05). ②The prenatal mental tension, fear and anxiety were significantly lower than those in control group(p<0.01). ③The degree of labor pain was significantly lighter than that in control group, The ratio of drug analgesia was significantly less than that in control group(p<0.01). The time of birth process was shorter than that in control group(p<0.01). The ratio of natural childbirth was significantly higher than that in control group(χ2=121.901, p<0.01). The ratio of cesarean section was significantly lower than that in control group(p<0.01). The total rates of operations including episiotomy, fetal head aspiration and cesarean section were significantly lower than those in control group(p<0.01). ④The ratio of uterine inertia was significantly lower than that in control group(p<0.01). ⑤The incidence of postpartum hemorrhage was lower than that in control group(p<0.05). The rate of neonatal complications, such as fetal distress, neonatal asphyxia and pneumonia etc., were significantly lower than those in the control group(p<0.01). Neonatal hypoxic ischemic encephalopathy rate was lower than that in control group(p<0.05). The neonatal mortality rate was less than that in control group, but no significant difference(χ2=2.007, p>0.05). The total incidence of maternal and infant complications was lower than that in control group(p<0.01). ⑥The pure breastfeeding rate was significantly higher than that in control group(p<0.01). ⑦ The skills of nursing care of newborn, bath a newborn, newborn touch were significantly better than those in control group(p<0.01). ⑧ The postpartum mood was obviously better than that in control group(p<0.01); The rate of postpartum depression was significantly lower than that in control group(p<0.01). Support of the family system was significantly higher than that in control group(p<0.01). ⑨ The maternal postpartum hemorrhage, postpartum depression, postpartum infection, mastitis occurrence rates were lower than those in control group(p<0.01). The satisfaction of patients was significantly higher than that in control group(p<0.01); the complaint was significantly less than that in control group(p<0.05); the incidence of medical disputes is lower than that in control group, but the difference was not statistically significant(χ2=2.007, p>0.05). Baby 6 months, suffering from acute infection of the upper respiratory tract, diarrhea, vitamin D deficiency were significantly lower than those in control group(p<0.01). Infants’ physical and mental development was better than that in control group(p<0.01). ConclusionsThis study through the analysis and discussion for many years has been a threat to maternal and infant health and life safety of a series of serious problems, for a variety of effects of natural childbirth, maternal and infant complications caused by biological and social risk factors, actively looking for and take the necessary preventive measures, in order to improve the pregnancy outcome, the promotion of maternal and child health. Based on the observation group of 269 cases of pregnant women, the implementation of the system of "phsyco-prophylax-is analgesia" comprehensive clinical intervention measures, and compared with the control group, to observe the effect of clinical intervention of two groups of pregnant women in different. The results show : the group of maternal health care awareness, compliance medical treatment increases, can ensure the healthy pregnancy, spontaneous abortion rate decreased significantly; pregnancy well, effectively prevent various diseases during pregnancy. During childbirth, the pregnant women of observation group can effectively come true phsyco-prophylax analgesia, promote natural delivery, reduce the rate of operation, reduce and avoid the intrapartum maternal and infant complications, ensure effective breastfeeding infants, reduce infant illness, promote the infant physical and mental development, reduce puerperal disease, promote maternal rehabilitation, improve patient satisfaction, improve the doctor-patient relationship, achieved the goals of a series of anticipated, the clinical breakthrough non drug obstetric analgesia.
