Objective:To explore the clinical application of minimallyinvasivetechnique in gastrointestinal abdominal emergency operation in elder patients.METHODS:The elder patients with acute abdomen who undergo surgery operateion in The General Hospital of PLA from January 2006 to June 2014were divided into laparoscopic groups(138 case) and laparotomy groups(170 case).on two groups of patients with the disease, operation time, bleeding volume, postoperativerecovery time, hospitalization time, complications and other indicators were statistically analyzed.RESULTS:The laparoscopy group and the laparotomy group showed statistically significant differences (P<0.05) in hospital stay[(8.5±7.5) d vs (11.5±9.9) d], off-bed activity time of postoperative[(20.6±7.7)h vs(35.2±5.6)h], rescovery time of postoperative gastrointestinal function[(26.9±9.1) h vs (39.3±10.6) h], blood loss[(67.1±47.7) ml vs (143.6±106.5) ml],and postoperative complications rate (4 vs 22).Analysised with different disease types, to acute appendicitis,upper digestive tract perforation,intestinal obstruction,The laparoscopy group and the laparotomy group showed statistically significant differences (P<0.05)in hospital stay[(4.7±2.4) dvs (7.6±3.4) d; (7.8±2.3) d vs(13.3±9.4) d; (8.1±2.1) d vs (12.8±8.1)d]、 off-bed activity time of postoperative[(16.0±3.9) h vs (26.1±7.0) h; (18.9±3.8) h vs (28.2±4.0) h; (23.8±3.6) h vs (32.9±4.2) h]、rescovery time of postoperative gastrointestinal function[(22.3±5.7) h vs (35.2±10.1) h;(28.3±5.7) h vs (39.9±7.8) h; (34.7±5.1) h vs (49.1±6.1) h]、blood loss[(34.3±24.0) ml vs (99.4±44.3) ml; (58.8±26.1) ml vs (139.0±61.0) ml; (75.4±22.1) ml vs (170.8±38.8) ml].Butperforation of small intestine,perforation of colon, mesenteric vascular disease,There was no statistically significant difference.Conclusions:Compared with laparotomy, the laparoscopy offers the advantages of less trauma, faster recovery, less hospital stay,and a lower postoperative Complicat-ions(4 vs 22) rate.laparoscopy is safe and effective in treating elder acute abdomen, which is worth popularizing in conditional hospital. But for acute appendicitis, perfo-ration of upper digestive tract, small bowel obstruction in patients with laparoscopic operation has more advantages.
