| Objective: Evaluate the clinical effect of Perthes disease after containment treatment which combined with triple osteotomy of pelvic and proximal femoral.Methods: Forty cases had their containment treatment in our department during May 2010 to December 2012. Their are 29 males and 11 females, aged from 6 to 10.4 years old(average, 8.3 years). All patients separated into two groups and each group has 20 patients. Group A had triple pelvic and proximal femoral osteotomy and group B had other containment surgery(proximal femoral osteotomy, Slater pelvic osteotomy or combine surgery). Radiography analyzes before and after surgery between two groups and Harris score in the last follow-up are performed. Each of measurement changes are evaluated as their effective in the containment treatment.Results: All patients get follow-up for an average of 32 months(from 18 to 48 months). All cases have had satisfactory outcome as their femoral head had well covered within acetabulum. Compare with two groups, Group A achieved more increase in CE angles, more reduces in SFR and Sharp angle, more coverage of femoral head.(P<0.05). Harris score shows a little higher in Group A.Conclusions: Our study shows triple pelvic osteotomy combine with proximal femoral osteotomy could achieve well containment effect in the Perthes disease even in some severe cases. Compare with other surgery, this method could effectively prevent the complications caused from single operation which may need more bigger correction in the local areas. |
