Objective: To observe the therapeutic effect of human umbilical cord Wharton`s jelly-mesenchymal stem cells transplantation on heart inflammation with heart failure of acute myocardial infarction. Methods: Experiment research: Rat models of heart failure of acute myocardial infarction were made by isoprenaline(ISO),40 survival rats were randomly divided into human umbilical cord Wharton`s jelly-mesenchymal stem cells transplantation group and normal control group after the success the model,sham group was made of 12 health rats, and then each of the three groups was subdivided into pre-transplantation group and post-transplantation group 4 weeks later. human umbilical cord Wharton`s jelly-mesenchymal stem cells transplantation group was transplanted with human umbilical cord Wharton`s jelly-mesenchymal stem cells with DAPI labeled after ISO injected one week. Sham group and normal group were untreated and normally bred. Echocardiography examination was performed before transplantation and post-transplantation 4 weeks laterto measure left ventricular ejection fraction(LVEF), Immunohistochemistry and immunofluorescence and ELISA were performed to investigate the injected cells and the expression of TNF-α. Clinical research: Methods to retrospecctive analyze the clinical data of heart failure of acute myocardial infarction patients in Chinese PLA 303 Hospital,which was to be divided into transplantation group(37) and control group(37).Transplantation group with conventional medical and transplanted with human umbilical cord mesenchymal stem cells,control group received conventional medical treatment.Analysis the plasma NT-pro BNP, TNF-α in the peripheral blood and cardiac function was detected before transplantation and 14 days after transplantation and 2 months after transplantation. Severe adverse clinical events were recorded. Results: Experiment research: Compared to pre-transplantation group, human umbilical cord Wharton`s jelly-mesenchymal stem cells transplantation group increased the LVEF; Compared to pre-transplantation and normal control, human umbilical cord Wharton`s jelly-mesenchymal stem cells transplantation group reduced the TNF-α and NT-pro BNP in the serum(P<0.05)and the expression of TNF-α from the heart tissue(P<0.05).Pearson analysis demonstrated that serum TNF-α and NT-pro BNP showed statistically positive correlation(r=0.65,P<0.05); Compared to normal transplantation, human umbilical cord Wharton`s jelly-mesenchymal stem cells transplantation group reduced the mortality from 33.3% to 16.7%;Immunofluorescence demonstrated that transplanted cells were still found alive in the heart after transplantation 4 weeks later. Clinical research: No significant difference was observed in adverse clinical events during the treatment and follow-up periods between the two groups. In both groups, NT-pro BNP and TNF-α level was lowered significantly after the treatmentl,and the improvement was more obvious in the treatment group(P<0.05),a significant difference was found between the two groups;A significant difference was found in LVED between the before operations and after operations 2 months(P<0.05),no significant difference was found in other indexs.No significant change was found in all kinds of indexs of control group.No significant difference was found between the two groups.Pearson analysis demonstrated that serum TNF-α and NT-pro BNP showed statistically positive correlation(r=0.63,P<0.05).Conclusion: 1.Human umbilical cord Wharton`s jelly-mesenchymal stem cells therapy of heart failure of acute myocardial infarction may be effective and not increase the risk. 2. Heart inflammation may be improved by WJ-MSCs transplantation inhibit the TNF-α from circulation and heart tissue. 3.The WJ-MSCs transplantation can ruduce the NT-pro BNP level on heart failure of myocardial infarction..4.The level of heart failure may be reflected by TNF-α.
