| Objectives:To evaluate diagnostic value of multifunctional specimen collector in patients with lung cancer;To evaluate the effect of aspiration before and after the transbronchial biopsy on the positive diagnostic rate of the visual lung cancer under endoscopy;To assess the effect and safety of aspiration in high-risk patients of biopsy under endoscopy.Methods:There are three stages:The first stage : To evaluate diagnostic value of multifunctional specimen collector( patent number: ZL201120168251.2) in patients with lung cancer,302 cases of patients in out-patient and inpatient department for suspected lung cancer from October 2012 to January 2014 were entered prospectively into the study, including 28 cases of non lung cancer in the final diagnosis. Transbronchial biopsy and multifunctional specimen collector successively were performed in 130 cases of lung cancer which could be observed directly by bronchoscope. Of non-visible tumours by transbronchial 68 cases were performed by transbronchial lung biopsy and multifunctional specimen collector successively,36 cases were performed directly by bronchoalveolar lavage, and 40 cases were performed by multifunctional specimen collector lavage.The second stage: To evaluate the effect of aspiration before and after the transbronchial biopsy on the positive diagnostic rate of the visual lung cancer. Under endoscopy aspiration before biopsy, biopsy, aspiration after biopsy were performed successively in patients with visible tumors in the endoscopic and suspected lung cancer from September 1,2013 to September 1,2014 in the respiratory endoscopy room of Shao Yang Central Hospital. All the selected patients at last were diagnosed with lung cancer by biopsy or cytology, surgical operation. The positive rate of aspiration was compared between before and after biopsy.The last stage:To assess the effect and safety of aspiration in high-risk patients of biopsy under endoscopy. 57 cases of patients in the respiratory endoscopy room of Shao Yang Central Hospital for suspected lung cancer from April 2014 to March 2015 were entered prospectively into the study,this patients are high-risk patients of biopsy under endoscopy, or have relative contraindications in biopsy.Results:The first stage:Transbronchial biopsy and multifunctional specimen collector successively were performed in 130 cases of lung cancer which could be observed directly by bronchoscope. Of non-visible tumours by transbronchial, 68 cases were performed by Transbronchial lung biopsy and multifunctional specimen collector successively, 36 cases were performed directly by bronchoalveolar lavage, and 40 cases were performed by multifunctional specimen collector lavage. The diagnostic positive rate of lung cancer were as follows: 80.0%(the positive rate of using the multi-function specimen collector attract and Transbronchial biopsy was 88.5%), 55.8%(the positive rate of using the multi-function specimen collector pump and transbronchial lung biopsy was 58.8%), 19.4%(7/36), 45%(18/40).The second stage:In 163 patients,142 patients were diagnosed with lung cancer at last, including 92 males and 50 females with mean age(62±21 years).Among 142 patients,there were 101 cases with massive changes and 41 cases with diffuse infiltrative changes. 52 cases(36.6%) were confirmed by aspiration before biopsy and 66 cases(46.4%) confirmed by aspiration after biopsy. Sub group analysis: for massive changes, the positive rate of aspiration before and after biopsy were 24.7%(25 cases),48.5%(49 cases),respectively; for diffuse infiltrative changes, the positive rate were 65.8%(27cases),41.4%(17 cases), respectively.The last stage:57 cases of patients were entered prospectively into the study. The positive rate of aspiration were 63.15%(36 cases).and the positive rate of common sputum cytology were 12.28%(7 cases).23 cases were bleeding in aspiration, but only a little. Conclusions:The positive rate in the diagnosis of lung carcinoma can be increased by the combined multifunctional specimen collector. The positive diagnostic rate of the visual lung cancer under endoscopy aspiration before and after the transbronchial biopsy is different. The safety of aspiration under endoscopy using multifunctional specimen collector were proved in the lung cancer patients of diagnosis. |
