Background Lumbar intervertebral disc herniation is one of the common causes of low back pain, see more adults, mostly young adults between 20 to 40 years old, in recent years gradually younger trend. DR-PACS-HIS system a variety of post-processing function, provides the diagnostic doctor read and abundant information. Especially the skeleton pixel values of the application, quick, simple, easy to promote, DR is photography bone lesions in the early detection, diagnosis, and one of the important methods of screening. In the degeneration of lumbar endplate damage is the main cause of intervertebral disc degeneration, some scholars studied the end plate thickness associated with bone mineral density, found that the thickness of the endplate and bone mineral density, distribution between different vertebral endplate and the same differences between different vertebral endplate. How to CT and DR-PACS-RIS system together, in patients with acceptable to achieve optimal economy and inspection results, will be the future research needs to solve an important problem, especially in patients with claustrophobia, DR-PACS-HIS system and CT is one of the better methods used.Objective To explore the clinical value of Digital radiography-Picture archiving and communication system-Hospital Information System (DR-PACS-HIS system) and Multislice Computed Tomography(MSCT)in lumbar disc herniation.Methods Of the 44 lumbar disc herniation from 2013 to 2014 in our hospital with 25 males and 19 females,the age is from 24 and 84 and average 53.4 years;44 patients with 220 vertebral segments and 176 lumbar disc segments.Radiographs,MSCT and Magnetic Resonance Imaging(MRI) of lumber spine were performed on each case and the images were input DR-PACS-HIS system and personal computer. Oberseving the whole Lumbar disc structure of spine and measure data which including,the upper and lower edges of the vertebral body and the CT value of pixel values, the intervertebral space height in DR, MSCT and MRI, the width of the spinal canal DR observe the extent of disc herniation, vertebral upper and lower edge MRI signal change. The MRI signal change of vertebral endplate and Fardon standard disc were divided into three groups (mild, moderate and severe groups), (Modic I, II, III three groups), evaluate DR-PACS-HIS systems and MSCT value in lumbar disc.The Statistical analysis is implied.Using for ststistical analysis package SPSS 13.0,p< 0.05 shows stationtical significance.Using spearman correlation and kruskal-wallis analysis.Results There were 92 segments intervertebral space height narrowing,26 segments before and after the narrow width of the intervertebral space height. There were 7 segments of L2/3 disc space narrowing,15 segments of L3/4 disc space narrowing,39 segments of L4/5 disc space narrowing,57 segments of L5/S1 disc space narrowing. There were 92 segments were lumbar disc herniation,,which left lumbar disc herniation 11, right lumbar disc herniation 12, e medium-sized lumbar disc herniation was 69. There 86 segments(48.89%) intervertebral disc herniation, L3/4 disc (6.82%), L4/5 intervertebral disc (18.18%), L5/S 1(21.02%). Mild disc prominent group (11.36%), moderate prominent group disc segment (36.93%), and severe disc prominent group (0.56%). Statistical analysis showed that the results of DR-PACS-HIS MRI and CT disc three levels of data does not have to meet the normal distribution, was used to compare kruskal-wallis rank sum test.1 and 2 groups of pixel values and CT value was statistically significant.There were 44 cases of 220 vertebral body and 191 vertebral bone hyperplasia, 6 cases Schmorl nodules,126 vertebral segments sclerotic rim edge. There were 191 segments of vertebral bone hyperplasia,6 cases Schmorl nodules. There were 162 vertebral bone hyperplasia,60 vertebral edges appear Modic changes, type 120 (33.33%), type â…¡ 38(63.33%), typeâ…¢ 2(3.33%). Three sets of data do not satisfy the normal distribution, using kruskal-wallis rank sum test, three sets of pixel values and CT value no difference was not statistically significant.Conclusion DR-PACS-HIS system and MSCT evaluate have statistical significance, which can be used for lumbar disc herniation evaluation, DR-PACS-HIS system and MSCT for Modic change evaluation was not statistically significant.The evaluation of Modic changes needs further study. |
