| Objective:To understand the situation of occult hepatitis B virus infection(OBI),and the molecular evolutionary characteristics of OBI virus S gene, and the influent factors of hepatitis B vaccination non-response in 1-year-old population. To providing reference for the prevention of hepatitis B virus infection in infants.Methods: Using continuous collect samples for cross-section survy. Collecting samples who completed the whole hepatitis B vaccination when it do 1-year-old physical examination during Septemper 2013 to August2014 in the preventive care&vaccination department of First Affiliated Hospital of Xinjiang Medical University.Getting structured questionnaires of infants while collecting venous blood samples with their relative consent, Epidata3.1 software for double double over the strategy of the original data enter and verification checked.Statistical analysis for SPSS15.0.ELISA methods detected serum HBV markers, get amplificated of HBV S gene by nested PCR.Analysis the characteristics fragment molecular evolution by MEGA6.06 if it was PCR(+).Results: 646 objects in this survey,â‘ the OBI infection rate was 8.67%(56/646). 42(75.00%) cases were genotype C and 14(25.00%) cases were genotype D in56 cases OBI strains. 13(30.95%) cases adrq+ serotypes and 29(69.05%) cases adw2 serotypes in genotype C strains. All of 14 genotype D virus strains are ayw2 serotypes. C genotype strains occur T148 I once, D genotype strains occur S114 P 5 times, P120 A once and G159 A once in the HBV amino acid “major hydrophilic regionâ€.â‘¡ The rate of hepatitis B vaccine non-response is 18.42%(119/646). The study found breast feeding≥6months(OR=0.532,95%CI:0.312~0.907)is the protective factor to HBV vaccinenon-response and father’s operation history(OR=2.627,95%CI:1.367~5.048) is the risk factor to HBV vaccine non-response. Not found the relation between OBI and HBV vaccine non-response.Conclusion: â‘ There is a certain proportion infected OBI in1-year-old population. C genotype is the possible dominant genotype.OBI strain occur T148Iã€S114Pã€P120A and G159 A in “major hydrophilic regionâ€.They could change the antigenicity and immunogenicity of HBsAg. And we should pay more attention to the prevention of OBI. |
