| Purpose:According to the anatomical features of anterior lumbosacral segment, to design a anterior internal fixed system which can go through iliac blood vessels and across any segment of L4-S2. Method:According to the measure of the lumbosacral segment(L4-L2) anatomical of 34 Chinese adult corpses, to conclude the safe operation area of anterior outer of lumbosacral segment and the breadth/area of the iliac blood vessel furcation/junction, then to analyze the affect that the height of the iliac blood vessel furcation/junction will be to the safe operation area of anterior lumbosacral segment. According to all of the measure result, to design a anterior internal fixed system which can go across segment of L4-S2. Result:(1) The anatomy structure of the anterior lumbosacral spinal cord is complex, covering aorta abdominalis, postcava and the common iliac arteriovenous. However, there is enough safe region for the operation and embed the internal fixation devices. After the anatomy measurement of the thirty-four Chinese anatomical specimens, we find that there is a safe operation room below the L4 to S2 and the iliac blood vessel. The covering of the iliac blood vessel deformation has a great influence on the area and shape of this region.It can accomplish the operation and the placement of the internal fixation devices using different operation by taking the good advantage of operation room and proper internal fixation devices. However, the difficulty and risk of different types of operation is not the same.(2) According to anatomic study of the lumbosacral anterior, an dead locked forward tacking system has been designed which is less affected by the position of iliac blood vessels and can be done in L4-S2 to process the long distance, single segment or cross segment fix.Conclusion:(1)The lumbosacral segment(L4-S2) safe operation area includes anterolateral of low back and sacral segment which the area is broad enough and can complete most internal fixation operation to use this area and choose the suitable internal fixation apparatus.(2)Based on the research of the anterior lumbosacral spinal cord, we design an anatomy screw-plate system on anterior lumbosacral spinal cord which is less influenced by the__ of iliac blood vessel and can embed in L4-S2, accomplish the long distance, the fixation of single-level or cross segment as well as has the advantage of anti-back, anti-lock and universal.(3)The anatomy screw-plate system on lumbosacral spinal cord has the advantage of long distance, single-level or multi-level, universal and anti-back. Besides, it can be applied to wide operative indications, which are the vital optional equipment of internal fixation on the anterior lumbosacral spinal cord and make the solution to the shortcomings of the use on U-reconstruction plate possible. |
