Epidemiological Investigation Analysis Of Chroni Obstructive Pulmonary Diseases Of Li And Han Ethnic Group

Objective: To explore prevalence rate distribution of Li People population, analyze and compared correlated risk factors of COPD between Li People and Han People population, explore the expression and meaning of VEGFA, VEGFD and HIF-1α in serum affecting on chronic COPD of Li People population, and reveal rhe associativity between different clinical staging and serum VEGFA、VEGFD and HIF-1α.Methods: With cross-sectional investigation method to investigation prevalence distribution of Li People population, chose 212 cases of COPD as case group, and 236 cases of non-COPD as control group. With stratified cluster sampling with unequal proportion method, chose 291 cases of COPD and 301 cases of non-COPD of Li People population from south central of Hainan Province, then collected 277 cases of COPD and 307 cases of non-COPD of Han People population from Hainan Province. Correlated risk factors of COPD in Li People and Han People were respectively analyed with Case-Control study method, then compared the difference about risk factors between the two peoples. ELISA method was used to detect detect the levels of VEGFA, VEGFD and HIF-1α factors in serum. According to the layers of peoples, sex and smoking, the expression difference was analyzed between the case group and the control group, and analyzed the correlations between clinical staging of COPD and differenct serum factors.Results: Li People in some ereas of Hainan the prevalence of COPD was 5.07%(286/5637). Multiple factors analysis result showed that the risk factors of COPD included in age(OR=1.096), expectoration(OR=87.917), locomotor activity limitation(OR=3.908) and frequency of respiration(OR=2.512), but BMI(OR=0.893) and a second rate(OR=0.000) were the protective factors of COPD. Han People population: Multiple factors Logistic Regression results showed that age(OR=1.052), frequency of breathing(OR=3.193), surrounding pollution of habitation(OR=1.574), smoking(OR=5.230), length of smoking(OR=1.965), repeated respiratory infection(OR=1.052) and family history of respiratory system disease(OR=16.035) were the risk factors of the incidence of COPD, but average living area was the protective factor of COPD. Li People population: Multiple factors result showed that age(OR=1.096), low BMI(OR=3.006) and recurrent respiratory tract infection(OR=47.785) were the risk factors of COPD. In the population of Li People, the levels of VEGFA and VEGFD in the case group were much higher than the control group(P<0.01). In the case group, the levels VEGFA and HIF-1α in Li People were much lower than Han People(P<0.05). In the population of Li People, the levels of VEGFA and VEGFD with male and female in the case group were much higher than the control group(P<0.05). In the population of Li People, the levels of VEGFA and VEGFD with smoking and non-smoking in the case group were much higher than the control group(P<0.05). The levels of VEGFA in acute stage and paracmastic stage were much higher than the control group(P<0.05), and the level of VEGFA in acute stage was also higher than paracmastic stage. The level of VEGFD in acute stage was obviously higher than the control group(P<0.05). The level of HIF-1α in paracmastic stage was higher than the control group(P<0.05), and the level of HIF-1α in acute stage was higher than paracmastic stage(P<0.05). There was positive correlation between COPD staging and VEGFA(r=0.405,P<0.01), and also positive correlation between COPD staging and HIF-1α(r=0.174,P<0.01).Coclusions: Li People has a lower prevalence of COPD in Part of Hainan, Most of this population exposed in the same environment factors, it hints that the influence of environmental factors is relatively small, and the effect of genetic factors is relatively large. The high risk factors of Han People population included in high age, high frequency of breathing,living area exiting pollutions of fume or dust, small area of living, smoking and length of smoking, repeated respiratory infection and family history of respiratory system disease. We should adopt comprehensive intervention measures to aim directly at these high risk factors, to reduce the prevalence rate of COPD. The high risk factors of Li People population included in high age, low BMI and recurrent respiratory tract infection. The levels of VEGFA and HIF-1α in the COPD population of Li People were much lower than the population of Han People. But the indexes of sex and smoking play fairly less effect of VEGFA, VEGFD and HIF-1α on COPD population of Li People. ⑸The levels of VEGFA and HIF-1α in acute staging of COPD show stepping up in some extent, and it exists positive correlations between COPD staging and different factors. ⑹The incidence of COPD in Han People population is closely related with environment factors, but the incidence of COPD of Li People population is comparatively correlated with genetic factors.
