| [Background]Discipline construction is the basic work of hospital development with strategic significance. Development of a hospital by discipline construction is vital to hospital survival. All in all, the focus of the competition among hospitals is the medical disciplines and talents. Training and introduction of critical talents is vital to promote medical service and meet the needs of the health demand of regional masses. The research subject is a central hospital of a district in Shanghai (abbreviation:the A hospital), the study objective is to confirm the direction of discipline and talent construction and establish medical discipline and talent target system of the hospital in the Twelfth Five Year Plan period or the medium and long term (5-10 years) to optimize the allocation of resources and correspond to the development of regional socio-economic and the health demand of regional masses better.[Objective]To analyze the status of socio-economic of one district in Shanghai (abbreviation:the A district), the medical service efficiency, discipline and talent construction status of the Central Hospital of the district (the A hospital), make suggestions on medical human resources and discipline construction of the A hospital, provide the reference and theoretical basis for the further development of discipline and talent construction in the Twelfth Five Year Plan period or the medium and long term(5-10 years).[Contents]1. Data sources:From the official documents and statistics, inquired and drew the data and indicators which could accurately reflect the status of the socio-economic, medical service efficiency and human resources and discipline construction of the A district in Shanghai.2. Qualitative interview:Using experts in-depth interview and field survey, to get to know the comments and suggestions on medical human resources and discipline construction.3. Questionnaire survey:According to the research subjects and Objective, designed questionnaire and improved by pre-survey and expert consultation, then conducted the survey; By analyzing the accomplished questionnaire, came to the quantitative and qualitative conclusion.4. Contrast analysis method:Compared the medical discipline and talent construction status of the A hospital with other 4 secondary general hospitals in Shanghai which were upgraded to tertiary hospitals in 2010, identified the difference and made suggestions.5. Data analysis:Qualitative data were analyzed by content analysis. Database was setup using Epidata2.0 and Exce12007,and quantitative data were analyzed by SPSS13.0 and Exce12007.[Results]1. In the A district, the household population, the aging population And the highly-educated accounted for a higher proportion of the population, thereby the health demand of the residents is greater and they need better medical service.2. For the A hospital, the basis of discipline is week, the talent team is not adapted to the requirements of development. Most of the existing key disciplines are belong to the lower levels. The level of new technique is also lower. And the discipline characteristics are not obvious. The development of medical teaching and research is imbalance. The quantity of scientific projects and papers are not many, and not high level. Moreover, the hospital is lack of academic leaders with high academic status and reserve talented persons with promising potential.3. The A district government has increased investment in the A hospital and offered the preferential policies of talent introduction, especially supported the integration between the A hospital and TR hospital and the cooperation with Shanghai Jiao Tong University School of Medicine. All these will strongly promote the discipline and talent construction of the A hospital, bring the hospital unprecedented opportunities for development.4. The gap between the A hospital and other tertiary hospitals at the aspect of medical human resources and discipline construction is enlarged. Moreover, the obviously gap also exists by comparison with other 4 secondary general hospitals in Shanghai which were upgraded to tertiary hospitals in 2010. And the competition from the tertiary hospitals nearby, such as Huadong hospital, Huashan hospital and No.6 People’s hospital is also a great threat.[Recommendations]1. Integrate regional high quality medical treatment resources to adapt to regional social demands. Combine the A hospital and TR hospital into New TR Hospital and then strengthen the discipline and talent construction of the hospital. Promote the medical service efficiency of the hospital and make effort to be the veritable regional medical center.2. Clarify the direction of the development of the disciplines, identify the focus, distinguish primary and secondary and make plans of the discipline. Carry out non-equilibrium strategy of development for the sake of achieving breakthroughs in 2-3 discipline fields, and then promote overall level of the discipline of the hospital through construction of several key disciplines.3. Optimize the developing environment of discipline and give the hospital sufficient financial support and offer the preferential policies. In addition to the recombination of New TR Hospital, integrate the resources of the medical group and medical colleges, establishing a security mechanism of key discipline construction. Create a good external environment for the discipline and talent construction.4. Strengthen the talent construction and setup a perfect mechanism of talents training and introduction, establishing a talented person echelon with reasonable knowledge structure and age structure. Attach importance to selection of academic leaders and maintain sustainable development of disciplines.5. Innovative mechanism, improve the system, promote the development of discipline. Strengthen the leadership and management of the discipline construction. Standardize plans of key disciplines and talent training and the procedure of approving a research project. Set up the target responsibility system of the discipline and talent construction. Toughen up the daily management and periodic assessment of discipline and talent construction.
