The Feasibility And Cosmetic Benefit Of Laparoscopic Gasless Single-Site Operative Platform

Objective:To explore the feasibility and cosmetic benefit of a self-designed laparoscopic gasless single-site operative platform,providing technical and theoretical basis for the development of laparoscopic gasless single-site surgery.Methods:Establishing zoic trial model and taking photos of the operative space of pneumoperitoneum and laparoscopic gasless single-site operative platform under laparoscope respectively.Finishing laparoscopic cholecystectomy of pigs with the laparoscopic gasless single-site operative platform.Vital signs were monitored by ECG monitor and endotracheal intubation to support respiratory oxygen during the whole animal experiment.Recording animal experiment-related data:1.The operative space of pneumoperitoneum.2. The operative space,operating time,surgical bleeding,incision length and intraoperative complications of laparoscopic gasless single-site operative platform. Comparing the operative space of pneumoperitoneum and laparoscopic gasless single-site operative platform to evaluate the exposure of operative space under laparoscopic gasless single-site operative platform. Results:Accomplish5cases animal experiment,the vital signs of pigs were stable during the whole process.The animal experiment-related data:1.The operative space of pneumoperitoneum:the operative space is neat and smooth, can provide sufficient exposure of the operative field.2.Laparoscopic gasless single-site operative platform:2.1Operative space:The platform lifting the whole anterior abdominal wall well,can provide a sufficient exposure of the operative field as pneumoperitoneum.2.2Operating time(including space setting and gallbladder removing two parts):70min(35min and35min),54min(22min and32min),50min(20min and30min),42min(12min and30min) and55min(20min and35min),the median is54min(20min and32min).2.3Surgical bleeding:10ml,14ml,13ml,11ml and12ml,the median is12ml.2.4Incision length:40mm,37mm,35mm,34mm and36mm,the median is36mm.2.5Intranperative complication:part of greater omentum and intestinal canal got stuck on the umbrella leaf when the platform was folding,the folding angle of umbrella leaf are sharp and injuried the anterior abdominal wall to some extent.Conclusion:1.The preliminary outcomes suggested that the laparoscopic gasless single-site operative platform is feasible in gasless single-site laparoscopic surgery,also having some cosmetic benefit.2.The issue that omentum and intestinal canal got stuck on umbrella leaf, injuried anterior abdominal,etc.needed to be solve in time.
