Canalith Repositioning Procedures In Treatment Of2118Patients With Benign Paroxysmal Positional Vertigo

Objective Collected data of the cases with diagnosis patients from January2008to December2013in the first affiliated hospital of guangxi medical university otolaryngology-head and neck surgery diagnosis and treatment data of2118patients with BPPV curative effect analysis.Methods2118patients with BPPV were treated with the Fast three steps modified canalith repositioning Epley reset method and Barbecue reset method to were retrospectively studied After the treatmentall patients were followed up for6months. Results①Of the2118patients,971were male and1147female,with a mean age of49.36+11.73(8to87years old),Lesion was located in the posterior semicircular canal in2005patients (94.66%), horizontal semicireular canal in88patients(4.16%),anterior canal semicireular in14patients(0.66%), combined semicircular canal in11patients (0.52%)②2005patients with PC-BPPV were divieded into two groups,1366patients with traditional Epley maneuver, the effective rate of the first time was90.6%,long-term (six months) efficiency was93.8%,639patients with the fast three steps modified canalith repositioning the effective rate of the first time was89.8%,long-term (six months) efficiency was95.8%.No significant difference between two groups of long-term efficiency(χ2=3.293, P>0.05);③At six months following up,the group of traditional Epley maneuver with a19.3%recurrence rate and the group of the fast three steps modified canalith repositioning was18.3%, No significant difference between two groups of recurrence rate(χ2=1.865, P>0.05);④Epley manipulative reduction relapse back to hospital to undergo full reduction treatment, accounting Epley manual reduction of100%of patients relapse, while the fast three steps modified canalith repositioning group was18.3%, statistically significant difference between the two groups(χ2=2.824, P<0.005);⑤Recurrence in patients,the proportion of recurrence,1or2times in modified canalith repositioning group and the fast three steps modified canalith repositioning group were43.4%、71.2%, the proportion of recurrence,3ormore than3times in two groups were56.6%、27.9%, Compare with the proportion of relapse frequency≥3times there is a significant difference (χ2=32.34,P<0.01)Conclusion The improved three-step approach quickly reset PC-BPPV treatment effect is obvious, its easy to operate, easy to learn, most patients can accept, and can reset their own treatment and the effect is better, is PC-BPPV patients with effective treatment.
