Objective: To have a preoperative comprehensive assessment ofmiddle-elders’ risks of having pulmonary embolism after thoracicsurgery, then use hazard classification for preoperative andintraoperative clinical intervention, thereby avoid the occurrence ofpostoperative pulmonary embolism as well as to improve the earlydiagnosis and treatment of pulmonary embolism level after thoracicsurgery.Methods: From January2008to December2012, our departmenthad a clinical research on585patients, which were classified into aCPM (Compound Preventional Measures) and a control group inaccordance with whether using clinical interventions. Measures:1. Acomprehensive preoperative assessment of the risks of havingpostoperative pulmonary embolism;2. Preoperative treatment withunderlying disease;3. Preoperative laxative, enema;4. Both lower limbsvascular USG before surgery;5. Fine intraoperative operation;6.Postoperative treatment of thrombus pump pressure treatment of lowerlimbs;7Blood gas analysis;8. Reduce or abandon the use of clottingdrugs after surgery;9Early using postoperative anticoagulation;10. Early postoperative ambulation, the incidence of postoperative deep veinthrombosis and acute pulmonary embolism in two groups wereobserved.Results: The control group contains356cases, in which215caseswere male,141cases were female, age range were from (49-70) years,the mean age was63.4years,26cases were found deep vein thrombosiswith the rate7.3%,17cases were found acute Pulmonary embolism withthe rate4.8%,2patients died and the mortality rate was0.5%. The CPMgroup contains229cases, in which138cases were male,81cases werefemale, age range was (48-71) years, the mean age was65.3years, deepvein thrombosis never occurred, one case were found pulmonaryembolism and the rate was0.4%.Conclusion: The implementation of targeted clinical interventionprograms is an effective way to reduce the incidence of thoracic surgeryin middle-elderly pulmonary embolism.
