| Objective:To evaluate the midterm effectiveness and lumbar protectionof postoperative lumbar disc herniation of young people withK-rod dynamic stabilization systerm via functional score andimaging examination.Methods:From December2010to July2014,51young patients withsingle-segmental lumbar disc herniation were surgically tr-eated by diskectomy with K-Rod dynamic stabilization syste-rm,including29males,22females,average40.3years old(ran-ge from19~58years old).The patients were followed up ran-ge from1~3years.The JOA score and VAS score were used to evaluate the preoperative,postoperative and midterm changes,the disc space heightã€intervertebal angleã€sagital verteb-rae slip and modic changes of operated segment and adjacentsegment were measured by imaging materials to evaluate theprotection to the spine with K-Rod dynamic stabilization s-ysterm. Results:(1)Preoperatevely the JOA score was14.83±1.75,comparewith the postoperative follow up time score(1monthã€6mon-thsã€1yearã€2years and3years:23.94±1.47ã€25±1.23ã€25.72±1.92ã€26.25±1.08ã€26.20±1.64),The JOA score were allobviously improved(P<0.05);Preoperatevely the VAS score was6.75±0.92,compare with the postoperative follow up time score(1monthã€6monthsã€1yearã€2years and3years:2.95±0.28ã€2.00±0.96ã€1.63±1.33ã€1.29±0.95ã€1.20±1.30),The VASscore were all notably reduced(P<0.05).(2)The intervertebral space height changes of operated s-egment and adjacent segment were followed up1monthã€3mo-nthsã€6monthsã€1yearã€2years and3years,the changes ofoperated segment space height score were0.31±1.76ã€1.18±1.42ã€0.49±1.63ã€0.78±1.51ã€0.14±1.93and0.42±0.83; thechanges of adjacent segment space height score were-0.33±1.56ã€-0.04±0.71ã€-1.11±0.95ã€0.99±1.47ã€0.59±0.61ã€-0.12±1.36.(3)Using flextion and extension radiography,the interve-rtebral angle and sagital vertebrae slip of operated segme-nt and adjacent segment were followed up3monthsã€6monthsã€1yearã€2years and3years;The intervertebral angle degree of operated segement were8.65±1.99ã€9.98±2.45ã€9.89±3.41ã€8.95±3.32ã€9.96±3.21; The intervertebral angle degreeof a-djacent segement were9.43±3.50ã€9.61±3.62ã€11.47±3.50ã€9.62±5.23ã€10.77±5.91,respectively compare with pre-operative intervertebrae angle degree(operated segment andadjacent segment:8.43±3.35and8.40±3.96),there is no important significance between follow up time and preoperation(P>0.05); The sagital vertebrae slip of operated segementwere1.36±0.74ã€1.67±0.86ã€1.99±0.83ã€1.71±0.26ã€1.21±1.27; The sagital vertebrae slip of adjacent-segement were1.28±0.60ã€1.48±1.35ã€1.90±0.60ã€1.48±1.03ã€2.42±0.87; respectively compare with sagital vertebrae slip (operatedsegmental adjacent segment:1.67±1.42and1.03±1.00),Thetest analysis indicated there is also no important signifi-cance(P>0.05).(4)Preoperative MRI revealed6cases suffered from endpl-ate inflammation (modic change),which were all classifi-ed1grade.3cases were all followed up to1year,one of t-he case were followed up to3years.Postoperative MRI indi-cated the modic change were notably improved. Conclusions:The midterm follow up revealed the single segment l-umbar disc herniation of young patients could be treatedwell by K-Rod dynamic stabilization systerm,which couldimprove clinical symptom effectivelyã€prevent degenerati-on of operated and adjacentsegementã€maintain lumbar in-stability and promote modic change reversibly. |
