The Cause And Treatment Method Of Anastomotic Bleeding After Radical Resection Of Right Colon Cancer

Objective:To observe the cause of anastomotic bleeding in radical resection of rightcolon cancer，explore its prevention methods during the operation and best wayto treat after operation.Method:Statistics195cases of radical resection of right colon cancer fromJanuary2011to June2014in our hospital,7cases of anastomotic bleeding（3cases during operation，4cases after operation）. Analysis the relationship withthe incidence of anastomotic bleeding and the factors which include thegender，the age，the operation method，the obstruction factors，the pathologicalstage. Summarize the intraoperative considerations to prevent anastomoticbleeding, At the same time we put forward the clinical diagnostic criteria ofanastomotic bleeding and the best treatment method.Results:1、The incidence of anastomotic bleeding in radical resection of rightcolon cancer is3.6%（7/195）in195cases，in which1.5%（3/195）duringoperation，2.1%（4/195）after operation。The incidence of men and women is4.46%（5/112）and2.41%（2/83），P＞0.05（χ2=0.581）.The incidence of≥55 and＜55is4.05%（6/148）and2.13%（1/47），P＞0.05（χ2=0.382）. Theincidence of open and laparoscopic surgery is2.78%（4/144）and5.88%（3/51），P＞0.05（χ2=1.049）. The incidence of with or without obstruction factors is4.35%（1/23）and3.49%（6/172），P＞0.05（χ2=0.043）. The incidence ofpathological stage I,II,III,IV is0%（0/10）、5.21%(5/96)、2.35%(2/85)、0%(0/4)，P＞0.05（χ2=1.624）.2、3cases were found active bleeding during operation and cured throughsuture the hemorrhagic spot.4cases were found hematochezia in3-6hoursafter operation and have different degrees of uncontrolled hemorrhagic shock,2cases cured through surgery with anastomotic resection,2cases cured throughendoscopic treatment with titanium clip.All7cases cured and there is nocontinuous bleeding until hospital discharge.Conclusions:1、The difference between the gender，the age，the operation method,theobstruction factors,and the pathological stage in anastomotic bleeding afterradical resection of right colon cancer has no statistical significance.2、In the treatment, endoscopic treatment is the first choice and next issurgical treatment, we should not choose the conservative treatment because itcan be life-threatening.
