BackgroundColorectal polyps are common and frequently-occurring digestive system diseases. Colorectal adenoma is considered as precancerous lesion of colorectal cancer. The splenic asthenia syndrome and the wetness-heat of large intestine syndrome are the two major syndrome types of colorectal adenomas according to Traditional Chinese Medicine syndrome differentiation. At present, the research about the rule and mechanism of syndromes in colorectal adenomas need depth studying, to reveal the intrinsic mechanism of colorectal adenomas, to guide the clinical treatment of Chinese Medicine and to improve the prevention and curative effect of Chinese medicine in colorectal adenomas canceration. Based on "the splenic asthenia syndrome arises from inherent defect and acquired disposition; the dysfunction of spleen in transport and transformation leads to the consequent stagnation of wetness and produce heat, and finally results in the stagnation of damp-heat pathogens in the large intestine" and "the phenomenon of normal colorectal mucosa—colorectal adenoma—colorectal carcinoma sequence", we detected miRNA express profiles of the two major syndrome types of colorectal adenomas using miRNA microarray to find out biology background and occurrence pathogenesis of TCM syndromes in colorectal adenomas.Objects105cases of patients with colorectal polyps were collected, from which280polyps were detected.115polyps samples were collected for testing which included75adenomas (including63tubular adenomas),20inflammatory polyps,16hyperplastic polyps and4adenocarcinomas. Another10normal colorectal mucosa biopsy samples obtained from adults who underwent colonoscopy for healthy examination were selected as control group. Among the63cases of patients with tubular adenomas,21cases belonged to the group of splenic asthenia syndrome,33cases belonged to the group of the wetness-heat of large intestine syndrome, and9cases belonged to the group of stagnation of qi and blood stasis syndrome. The biopsy samples were obtained from4cases of splenic asthenia syndrome,4cases of wetness-heat of large intestine syndrome and4cases of normal health examination people to research miRNA expression profile.ObjectiveIn this study, we detected miRNA express profiles of the two major syndrome types of colorectal adenomas and normal health people using miRNA microarray to find out biology background and occurrence pathogenesis of TCM syndromes in colorectal adenomas.MethodAll objects were examined by enteroscopy after intestinal tract clearing preparation by taking orally castor oil or clusis. The colorectal samples were collected from the polyp tissues and nomal colorectal mucosa. The miRNA express profiles were detected by miRNA chip technology.ResultsWe found254miRNAs that were significantly differentially expressed in the three groups:1. Compared with control group,204miRNAs were significantly differentially expressed in the wetness-heat of large intestine syndrome of tubular adenoma group. Among them,100miRNAs were up-regulated and104miRNAs were down-regulated.2. Compared with control group,61miRNAs were significantly differentially expressed in splenic asthenia syndrome of tubular adenoma group. Among them,32miRNAs were up-regulated and29miRNAs were down-regulated.3. Between the different types of TCM syndrome in tubular adenoma,138miRNAs were significantly differentially expressed. In contrast with the splenic asthenia syndrome group,76miRNAs were up-regulated and62miRNAs were down-regulated in the wetness-heat of large intestine syndrome group.4. Compared with control group,45miRNAs were significantly differentially expressed in the two tubular adenoma groups. Among them,17miRNAs were up-regulated and28miRNAs were down-regulated. Conelusions1.The abnormal expressed miRNAs may participate in the pathological process of colorectal tubular adenoma. It is an early important event in colorectal cancer and has very important significance for early diagnosis and prevention in colorectal tumor.2. The abnormal expressed miRNAs may participate in the pathogenesis transformation process of splenic asthenia syndrome to wetness-heat of large intestine syndrome of colorectal tubular adenomas, which reflects the evolution tendency of TCM deficiency and excess syndromes.3. Differentially expression of miRNAs between different syndrome types of TCM with the same histopathological type of colorectal tubular adenomas may be one of the molecular basis of TCM deficiency and excess syndromes.
