| Objective: Analysis of association between thedistribution pattern of TCM syndromes and BP,risk factor,AGT mRNAexpression and AGT methylation status in primary hypertension.It willhelp us to clarify the association betweenthe AGT gene with primaryhypertension occuring and somatotype TCM syndromes of primaryhypertension. Methods:(1)Design the questionnaire to statistics clinicaldata of participants.They were gender,age,stature, weight,BP and diseasecourse and so on.Analysis the constituent ratio,age regulation and diseasecourse regulation regulation of TCM syndromes with primaryhypertension(It include sthenic liver yang type,sthenic yang and yindeficiancy type,stagnation of phlegmatic hygrosis type and both yang andyin deficiency type).Comparison the BP and BMI of TCM syndromeswith primary hypertension between with control group.(2)Detection theblood fat and Hcy of participants in Yibin city second people’s hospitalclinical laboratory.Comparison the results of differentgroups.(3)Extracting total RNA in blood and testing AGT gene mRNAexpression of participants by real-time fluorescent quantitative PCR.Thencomparison the results of different groups.(4)Extracting DNA in bloodand analyzing the AGT gene methylation status in promoter region of participants by pyrosequencing.Then comparison the results of differentgroups. Results:(1)TCM syndromes distribution in162cases primaryhypertension were sthenic liver yang type53cases,sthenic yang and yindeficiancy type42cases,stagnation of phlegmatic hygrosis type41casesand both yang and yin deficiency type26cases.The proportion in eachgroup were32.72percent,25.92percent,25.31percent and16.05percent.(2)The average age of disease.The average age of sthenic liveryang type group was (50.74±9.93) years.The average age of sthenicyang and yin deficiancy type group was(65.48±8.21) years.The averageage of stagnation of phlegmatic hygrosis type group was(57.66±9.73)years.The average age of both yang and yin deficiency type groupwas(72.45±5.43) years.Both yang and yin deficiency type group’saverage age was highest.Sthenic yang and yin deficiancy type group’saverage age was second.Stagnation of phlegmatic hygrosis type group’saverage age was third.Sthenic liver yang type group’s average age wasleast.The difference between groups was statistically significant(P<0.05).(3)Disease course.The disease course of most patients in sthenic liveryang type group was less than five years.The disease course of mostpatients in sthenic yang and yin deficiancy type group was more than fiveyears.The disease course of most patients in stagnation of phlegmatichygrosis type group were five to ten years.The disease course of mostpatients in both yang and yin deficiency type group was more than ten years.(4)BP.Both yang and yin deficiency type group’s SBP was higherthan control group’s one.Aother three groups SBP and DBP wererised.The BP of sthenic liver yang type group was highest.The differencebetween groups was statistically significant(P<0.05).(5)BMI.Thenic yangand yin deficiancy type, stagnation of phlegmatic hygrosis type and bothyang and yin deficiency type groups’ BMI were higher than controlgroup’s one.The BMI of stagnation of phlegmatic hygrosis type groupwas highest.The difference between groups was statisticallysignificant(P<0.05).But the difference sthenic liver yang type groupbetween with control group was not statistically significant(P>0.05).(6)Blood fat.The situation about blood fat in primary hypertension groupsand control group.The blood fat was abnormal in primary hypertensiongroups.Stagnation of phlegmatic hygrosis type group’s TG,TC andLDL-C were higher than the other four groups,but HDL-C was lowerthan the other four groups.The difference between groups was statisticallysignificant(P<0.05).(7)Hcy.The situation about Hcy in primaryhypertension groups and control group.Primary hypertension groups’Hcywere higher than control group,and stagnation of phlegmatic hygrosistype group was highest.The difference between groups was statisticallysignificant(P<0.05).(8) The situation about AGT gene mRNA expressionin groups.Sthenic liver yang type group’s AGT gene mRNA expressionwas first.Sthenic yang and yin deficiancy type group’s AGT gene mRNA expression was second.Stagnation of phlegmatic hygrosis type group’sAGT gene mRNA expression was third.Both yang and yin deficiencytype group’s AGT gene mRNA expression was fourth.Control group’sAGT gene mRNA expression was lowest.The difference between groupswas statistically significant (P<0.05).(9)The situation about AGT genemethylation status in promoter region in groups.There were two CpGsites’MtI in five CpG siteswere different in five groups.Control group’sAGT gene MtI was highest.Both yang and yin deficiency type group’sAGT gene MtI was second. Stagnation of phlegmatic hygrosis typegroup’s AGT gene MtI was third.Sthenic yang and yin deficiancy typegroup’s AGT gene MtI was fourth.Sthenic liver yang typel group’s AGTgene MtI was lowest.Control group was high methylation status,butsthenic liver yang type group was low methylation status.Conclusion:(1)TCM syndromes of primary hypertension most are stheniasyndrome.Sthenic liver yang type’s constituent ratio in primaryhypertension is most.Sthenic yang and yin deficiancy type issecond.Stagnation of phlegmatic hygrosis type is third.Both yang and yindeficiency type is least.(2)The low age and new patients’ main syndrometype are sthenic liver yang type in sprimary hypertension.Old age andlong disease course patients’ main syndrome type are both yang and yindeficiency type in sprimary hypertension.(3)Both yang and yin deficiencytype only show SBP rised.Sthenic liver yang type,sthenic yang and yin deficiancy type and stagnation of phlegmatic hygrosis type’s SBP andDBP all rise,but sthenic liver yang type’BP rised ishighest.(4)Obesity,abnormal lipid metabolism and high Hcy are riskfactors of primary hypertension.These risk factors are highest instagnation of phlegmatic hygrosis type.They easy to cause cardiovascularand cerebrovascular events.(5)Primary hypertension patients AGT genemRNA expression higher than normal people.Sthenic liver yang type’sAGT gene mRNA expression in primary hypertension is most.Sthenicyang and yin deficiancy type is second.Stagnation of phlegmatic hygrosistype is third.Both yang and yin deficiency type is least.(6)There isassociation between AGT gene methylated modification with primaryhypertension and TCM syndromes of primary hypertension. |
