Effects Of Acupuncture And Guanyuan(RN4) Moxibustion On The Bone Repair Of Chronic Sinusitis Mice

Objective:To study the effect of acupuncture on the nasal mucosa surrounding bone tissues of chronic sinusitis rats. At the same time, to analyze that whether the therapeutic mechanism is related to the reduction of COX-2and MMP-2in order to provide a credible theoretical basis for treating chronic sinusitis with acupuncture in clinic.Methods:Using DPS13.1software completely randomly divided160male C57BL mice into8groups.This paper only incorporates the datas of acupuncture group,model group,sham operation group,normal group.Control group received no intervention.The control group received no treatment.Bacteria were not placed in the sham operation group after nasal drilling.Put small sponges with streptococcus infiltrating fluid into the bilateral maxillary sinuses of the rats in the remaining three groups to build chronic sinusitis models. In the western medicine group, the mice were treated with clarithromycin tablets0.103g/kg.d. In the acupuncture group, mice were treated alternately with either needling the bilateral Zusanli (ST36) and retaining the needles for20minutes or adopting Guanyuan (RN4) moxibustion for20minutes every other day. The conditions of the mice in the remaining groups without any intervention were recorded. The treatment lasted for14days. After the treatment, analyzed the nasal and sinus bone slices with HE staining and examined the expression of COX-2and MMP-2by immunohistochemical method.Results:After the treatment, there were no nasal obstruction and runny nose in sham operation group and the control group. In operation group, typical chronic sinusitis symptoms, tears and conjunctival congestion and edema were obvious. In acupuncture group and western medicine group, symptoms such as sneezing and runny nose were released significantly. In the control group, the sinus mucosa cilium epithelium structure of mice was normal. There was no inflammatory cell. In the operation group, the level of inflammatory was the most severe including inflammatory cell infiltration, disordered arranment of epithelial cells, necrotic epithelium mucosae, goblet cell proliferations and hyperplasia of glands and fiber tissues of submucosa. In western medicine group, the inflammation was better the operation group including less necrotic epithelium mucosae, lymphocyte and monocyte infiltration and hyperplasia of glands. In acupuncture group, the mucosa subepithelium was repaired obviously. At the same time, the inflammation was lighter than the control group and operation group. In sham operation group, the bone tissues, the arrangement of bone and fiber tissues was normal in the control group. In the operation group, the fiber proliferation was significant (++) with a large amount of inflammatory cells infiltration (+++), regional neutrophil infiltration (++), slight abscess formation and partial necrotic bone tissues. In western group, the fiber proliferation was significant (+++) with inflammatory cells infiltration (++), small necrotic bone tissues, obvious neutrophil infiltration (+++) and mature abscess. In acupuncture group, the fiber proliferation was moderate (++) with medium inflammatory infiltration (++) and slight neutrophil infiltration (++). In sham operation group, both of the fiber proliferation and inflammatory infiltration were slight. The expression of MMP-2in the operation group was the highest (P<0.01). There was no difference among the acupuncture group, western medicine group and control group (P>0.05), the acupuncture group was lower than the western group (P<0.05). The expression of COX-2in the operation group was the highest (P<0.01). There was no difference among the acupuncture group, western medicine group and sham operation group (P>0.05). The acupuncture group and western medicine were lower than the control group (P<0.05).Conclusion:Acupuncture could reduce the amount of inflammatory cells, granular cells infiltration and proliferation of glands to release the inflammation significantly. At the same time, acupuncture could release the inflammatory damage of nasal mucosa surrounding bone tissues by reducing the fiber proliferation. What’s more, acupuncture could reduce the expression of MMP-2and COX-2to release inflammation. The therapeutic mechanism might be related to that the reduced expression of MMP-2and COX-2could alleviate the inflammatory symptoms as well as enhance the capacity of the body on resisting injury and repair the nasal bone tissues.
