Objective: To explore the effect of human urinary kallidinogenase(HUK) on collateralcirculation in patients with acute ischemic stroke, hopefully to provide evidences for itsclinical application.Methods:30patients, who were diagnosed as acute ischemic stroke and have been verified tohave severe stenosis or occlusion in internal carotid artery or middle cerebral artery withMRA in Department of Neurology, the First Hospital of Jilin University during October2013to November2014, were taken as test subjects. The subjects were set into2groups:19casesas observation group and11cases as control group. The two groups of patients were bothtreated with regular treatment, but in addition the observation group were treated with HUK0.15PNAU by intravenous drip once daily, for7days. NIHSS was measured before and afterthe treatment, mRS was measured after3months. PWI, MRA and TCD tests were performedbefore and after treatment, to observe the volume of ischemic penumbra(IP)(the volume ofpenumbra the volume of infarction), cerebral blood flow and the recovery of collateralcirculation.Results:(1) After treatment, the NIHSS scores of observation and control group decreasedrespectively(4.58±1.31)(2.64±1.43), with significant difference between the two groups(P<0.01);3months after treatment, the mRS scores of observation and control group were(2.00±1.05)(2.82±0.98), with significant difference between the two groups(P<0.05).(2)After treatment, the IP volume of observation and control group decreased respectively(61.99±29.91)(9.68±18.27), with significant difference between the two groups(P<0.01).(3)After treatment, the rCBF in ipsilateral of observation and control group were increasedrespectively(196.24±114.33)(54.30±92.56), with significant difference between the twogroups(P<0.01).(4) After treatment, the rCBF in contralateral of observation group wereincreased(48.35±103.09), and that of control group were decreased(68.99±175.94), withsignificant difference between the two groups(P<0.05).(5) After treatment, collateralcirculation of10cases(52.63%) was improved in observation group, which of3cases(27.27%) was improved in control group.Conclusion:(1) Human urinary kallidinogenase can significantly improve the impairmentand disability of neurological function on patients with acute ischemic stroke.(2) Humanurinary kallidinogenase can improve collateral circulation, increase cerebral blood flow andreduce IP volume.
