A coronary artery fistula (CAF) is an abnormal connection between a coronary artery and any of the4chambers of the heart or any of the great vessels (superior vena cava, pulmonary artery, pulmonary veins, or coronary sinus).CAF is a rare coronary artery disease, which can be classified as congenital CAF or acquired CAF. Fistulas originating from the right coronary artery are more common, while CAFs draining into the right heart structures are more common. CAF can cause coronary steal. Most patients are asymptomatic. Most common symptoms are angina and congestive heart failure. transthoracic echocardiography or transesophageal echocardiography (TEE) can establish the diagnosis. coronary angiography and computed tomography cardiac coronary angiography can show clear anatomic characteristics. The treatment of CAF is still controversial. Considering the severe complication, we recommend surgical or transcatheter closure. We report three patients with CAF. They were undergone surgical treatment in our hospital. The postoperative course was uneventful. With appropriate surgical approach of CAF, operation is effective.
