The Curative Effect Analysis Of Microvascular Decompression And Balloon Compression Decompression In The Treatment Of Primary Trigeminal Neuralgia

Objective: Sudy the treatment methods and curative effect of micorovascular decompression(MVD) and Percutaneous Microballoon Compression will of The Gasserian Ganglion Treatment(PMC) in primary trigeminal neuralgia(TN),to be better guide the clinical to further put forward the messures of the comprehensive Treatment of primary trigeminal neuralgia.Methods: A retrospective analysis of the clinical data of 153 patients with primary trigeminal neuralgia which collected from December 2012 to December 2013 in LiaoNing Province People’s hospital of the second neurosurgical,the 153 patients all taken the surgery treatment,and been followed-up.124 cases of primary trigeminal neuralgia were treated by Percutaneous Microballoon Compression will of The Gasserian Ganglion Treatment(PMC)(micro balloon compression technique group), 29 patients with primary trigeminal neuralgia were treated by microvascular decompression(microvascular decompression group).The rate of treatment effect 、complications and recurrence of two surgical methods were statistically analyzed, finally, the cases of MVD invalid or postoperative recurrence were accepted the treatment of PMC. The cases of PMC invalid, or recurrence,according to the patient’s wishes and the patient’s own condition, were taken the treatment of PMC or MVD.The overall cure rate of the two kinds of surgical treatment for primary trigeminal neuralgia were calculated.Results: A total of 153 patients been followed-up, ranging from 10 to 24 months follow-up time, taken CT or MRI, to exclusion of secondary trigeminal neuralgia.The microvascular decompression group: 29 patients all received follow-up, 24 cases of postoperative pain disappeared immediately, two cases of effective, 2 cases of pain relief,one case invalid,the invalid patient was taken the PMC to cure. After taken the cure of MVD,3 cases had hearing loss and tinnitus,one case recoveryed in five months,the other 2 cases had no change.2 cases had Late onset facial paralysis i, all recoveryed in 3 months, 1 case had incision infection,,1 case had cerebrospinal fluid leakage of incision,all cured after strengthening suture and anti-infection treatment, found no intracranial hematoma, no death, in addition, almost all of the patients have different degrees of postoperative headache, dizziness, nausea, vomiting, both by recumbent rehydration disappeared after symptomatic treatment, 1 case of postoperative recurrence of 13 months later, been cured by taken PMC.The PMC group: 124 patients,116 cases ’s postoperative pain immediately complete remission, 4 cases were markedly effective, 2 cases were effective, 2 cases invalid. 1 case again taken PMC not ease, another cured after taken MVD operation;All patients after operation with a certain degree of facial numbness and masticatory force weak, facial numbness returned within 1 to 2 years, masticatory muscle weakness recoveryed of 2-3 months, oral herpes and double vision recoveryed in 1 to 2 weeks, corneal discomfort recoveryed after appropriate treatment of ophthalmology of 1-3 weeks, no intracranial hemorrhage, no death.The pain relief overall curative effect of MVD and PMCgroup have no significant difference.The cure rate and complications of MVD and PMC group have no statistical significant difference. The recurrence rate of MVD group(4.8%) is lower than the PMC group ’s,but had no statistically significant differences between the two groups.Conclusion: MVD and PMC can alleviate PTN.In the treatment of primary trigeminal neuralgia, MVD and PMC two kinds of operative methods can complement each other, to improve overall efficiency, the rate of the cure and recurrence of the two kinds of operation have no different significance,at the same time, according to the wishes of patients, PMC is also a good choice for the patients who can’t tolerate surgery or the elderly patients whose basic condition is poor.
