| Objective:In order to investigate clinical symptoms, gender, age, pathological types, pathogenic site and treatment of the multiple primary cancer(MPC) associated with gastric carcinoma,and the differences with homochronous single primary gastric carcinoma,the articlestudys the clinical characteristics of 130 cases with MPC associated with gastric carcinoma,and compares them with the clinical characteristics of 130 cases of single gastric cancer, as well as deepening the clinician understanding of the MPC related to cancer of the gastric.So,the MPC will be found and treated in earlier time.Methods:From January 1st,2002 to January 1st,2015,a total of 7,540 patients with gastric carcinoma in our hospital. Collecting 130 cases with MPC associated with gastric carcinoma and 130 patients with the sole gastric carcinoma, and their clinical data was analyzed retrospectively.The counting material was analysed with Chi-square test.The counting material which theoretical expected frequency is less than 1 was analysed with Fisher test.The measurement material was analysed with the T-test, Analysis of Chi-square test with p<0.05 was considered to be statistical significance.Result: 1. The incidence of MPC associated with gastric carcinoma in gastric cancer was 1.7%, There were 123 patients with double MPC, 7 patients with three MPC,76 cases of synchronous primary cancer and 54 cases of metachronous primary cancer in which there were 35 patients found the second primary cancer after diagnosing the first primary cancer 5 years。Mean time between the first primary cancer and the second primary cancer was 69.1 months.2. The clinical manifestations of the patients with MPC associated with gastric cancer was similar to gastric cancer patients,whoes clinical manifestations was mainly epigastric pain and epigastric full unwell. However,stool,hematemesis,emaciation and weakness were more commonly on the patients with MPC,whose clinical manifestations were more serious.3. The male patients were more than the female patients in both the MPC associated with the gastric carcinoma and gastric cancer. The mean age of patients of MPC associated with gastric carcinoma was(67.0+9.8) years when gastric cancer was diagnosed,while the mean age of patients of gastric cancer was(60.9+12.3)years.The analysis of Chi-square test with p<0.05 was considered to be statistical significance,and the age of the patients with MPC associated with gastric carcinoma was older.4. Colorectal cancer was the most common type of primary cancel in other organs(30.7%),followed bylungcancer(14.5%),urothelial tract cancer(13.8%),esophagus cancer(9.7%), hepatic cancer(7.3%), breast cancer(4.0%),and so on.5. There were 13 patients with gastric MPC(GMPC),which pathological types in middle or low differentiated adenocarcinoma was given priority to.There were a total of 26 gastric lesions,gastric antrum in 9(36.4%),caudiac 8(30.8%),stomach bottom 7(26.9%),gastric body 2(7.7%).GMPC risk was mainly composed of gastric antrum, caudiac department.Conclusion:MPC associated with gastric carcinoma often occurs in older men,which includes manily double MPC. The clinical manifestations of the patients with MPC associated with gastric cancer is mainly epigastric pain and epigastric full unwell. Colorectal cancer was the most common type of primary cancel in other organs, followed bylung cancer,urothelial tract cancer,esophagus cancer, hepatic cancer, breast cancer,and so on.The main pathological types of the GMPC is middle or low differentiated adenocarcinoma. GMPC risk is mainly composed of gastric antrum, caudiac department. |
