Objective:To study the usefulness of copeptin as a rapid and reliable maker for diagnoseing acute myocardial infarction(AMI).To compare association of the copeptin concentrations with the severity of coronary lesion and Gensini scores.To assess the 3months prognostic value of copeptin in a group of patients with acute myocardial infarction.Methods: The current study enrolled 150 patients(aged 36 to 87 years old) with chest pain presenting to our hospital.They,re divided into as AMI group with 75patients(average aged 64.65±11.87). The second group consisted of 75 healthy persons(average aged 61.45±10.54).Measured and compared copeptin and cTnI using samples collected..Patients were followed up for 90 days.Results:(1)Patients with acute myocardial infarction had higher concentrations of cTnI [4.06(0.66,13.93μg/L)] and copeptin [13.70(10.28,28.23)pmol/L] at admission compared with those without(P<0.05);(2) The concentrations of copeptin in three coronary arteries lesions group[26.82(12.90,50.45)pmol/L] and left main lesions group[37.55(29.72,42.69)pmol/L] were both higher than that in one coronary artery lesions group[13.97(10.50,22.18)pmol/L] and two coronary arteries lesions group[11.18(6.93,14.81)pmol/L](P<0.05).(3)But there was no statistical significance compared with one coronary artery lesions group[13.97(10.50,22.18)pmol/L] and two coronary arteries lesions group[11.18(6.93,14.81)pmol/L];The difference of threecoronary arteries lesions group[26.82(12.90,50.45)pmol/L] and left main lesions group[37.55(29.72,42.69)pmol/L] was not statistically(P>0.05).(4) Results the plasma copeptin level in four Gensini points group, being 17.56±3.28、35.77±4.76、51.50±6.44、91.60±16.48, and there was also no statistical significance(P>0.05).(5)Incidence of MACE during 3 months, The plasma copeptin level was higher than that in none-MACE group(P<0.05). major adverse cardiovascular events(MACE) were recorded during the 3 months follow-up.Conclusion:The plasma copeptin level in acute myocardial infarction is at a higher level,and to some extent it can reflect the severity of coronary atherosclerosis.The copeptin level is closely correlated with the prognosis of patients with myocardial infarction.
