Cross-sectional Study On The Situation Of Community Basic Public Health Service Of Four Communities In Kunming

Objective:To understand the Current Situation of basic public health services of four communities in Kunming (Guandu, clouds, Golden, Fengning), and the Community residents’ awareness of basic public health services, utilization, satisfaction and the expectation of project. To analyze its influence factors, so as to make recommendations to promote in-depth development of basic public health services, in order to promote the purpose of the orderly development of community health service standards.Methods:Cross-sectional survey research of the epidemiological investigation and the access type way of questionnaire and interview method was used to collect quantitative and qualitative data. Self-made questionnaire was used to investigate the residents of the four communities and statistic analysis methods including statistical description, one-factor logistic regression, multiple regression analysis were used. Qualitative interviews was used to interview the staffs of communities.Results:1. The development of basic public health work of four community health service1.1 Situation of community health institutionsThree community health service centers did not have independent legal personality, in such aspects as personnel, finance were no independent autonomous power; the planning layout of four community health service centers were basic up to standard.1.2 Human resourcesBoth general practitioners number and public health personnel configuration of four community health service centers were less than the countries suggested nuclear standards.1.3 The basic public health services carry out for all peopleFor the health records, four community health service centers carried out for permanent residents within the jurisdiction, but there is only one center by filing rate up to the standard. Service of health education, four communities health service centers are given priority to with health education, but there existed material specialized, residents understand but execution is difficult.1.4 Carry out basic public health services in view of the focus groupsHealth care for children, management compliance rate of two centers reach the standard. Maternal care, status of building cards/cards compliance rate of two centers up to standard. Maternal postpartum visit rate up to standard of three centers and there was a center did not carry out postpartum visit. The elderly health management, management rate of center four centers did not reach. 1.5 Carry out basic public health services in view of the crowd chronic diseaseCommunity health service centers in four standardized management of blood pressure in patients with hypertension rate was 100.00%; standardized management for patients with diabetes rate three communities was 100.00%; rate of standardized management for patients with severe mental illness have three communities was 100.00%.2 Community residents’ awareness, utilization, satisfaction and expectation of basic public health services2.1 Socio demographic characteristicsThe objects of this survey were female, the 44 year old and 60-74 years old, married, high school and above, local accounts, from retirement and unemployment, the number of persons in the household 2-4 people, low-income families (1001-5000 yuan) based.2.2 Analysis on the knowledge of community residents’ basic public health serviceCommunity residents for the community basic public health service awareness rate of 36.09%. Univariate logistic regression results show that the influ encing factors of the satisfaction of basic public health service is the household registr ation (OR=1.896,95%CI=1.340-2.681) and culture degree OR=1.216,95%CI=1.023-1 .447, average monthly income of the family (OR=1.335,95%CI=1.074-1.660), family population (OR=1.335,95%CI=1.072-2.199),age(OR=1.335,95%CI=1.072-2.199),oc cupation(OR=0.443,95%CI=0.280-0.702),for the basic community public health servi ce awareness (OR=2.505,95%CI=1.844-3.403).Conclusions1 The layout of four community health service centers is basically reasonable, but the whole network is not formed, and the number of public health personnel is in sufficient.2 Four community health service center health records established inadequate coverage, the establishment of form a single, part of the file due to the filing methods affected the under a step to households as a unit of health information use. There is no comprehensive assessment of community focused disease and the special needs of the crowd to carry out education.Infectious disease prevention and control work carried out from the publicity and education, infectious disease report processing, hygiene supervision to a class of seedlings inoculated has a good foundation, but still need to strengthen the supervision of urban gaps in population immunity, wary of new infectious diseases and public health emergencies.The health management for the key people and the key disease still need to work out the regional work plan according to the district characteristic, accomplish the quantity of the task and balance the quality of the task. The district data submitted errors, the training of staff should strengthen, the correct submission of data is conducive to carry out information analysis and management.3 The awareness of basic public health services is disequilibrium. Service utilization is not comprehensive.
