The Clinical Related Research About The Efficacy And The Quality Of The Patients’ Life With The Effect By Yiqi Zhuyu And Lishui Decoction Of The Ruptured Lumbar Disc Herniation

Objective:To observe the curative-effect and the quality of the ruptured lumbar disc herniation patients’life with the effect by YiqiZhuyu and Lishui Decoction and surgery. Analysis the advantages and disadvantages about the conservative treatment of ruptured lumbar disc herniation.Methods:We collect 60 ruptured lumbar disc herniation patients with similar symptom from the outpatients and inpatients of Suzhou Hospital of Traditional Chinese Medicine between January 2014 to January 2015. According to their willingness, we devided the patients into TCM conservative group (30) and surgery group < 30).With the way of telephone interview and Clinical follow-up interview, we fill out the JOA score and the SF-36 form in three period (nonetreatment,3 months later with treatment,6 months later and 1 years later with treatment. We make statistical analysis to assess the curative-effect and the quality of the patients’ life in 8 dimensions (physical functioning, role physical health, bodily pain, general health perceptionsvitality, vitality,social functioning, role-emotional problems and mental health).Results:(1) Conservative treatment with YiqiZhuyu and Lishui Decoction and surgery were both effective, and they both can improve the patients’ JOA score (P<0.01).Surgery has a higher JOA score than conservative treatment after 3 months’ treatment (P<0.05),but there is no difference between conservative treatment and surgery in long-term(6 months～1 year later) (P>0.05). Surgery has a higher JOA improvement rate than conservative treatment after 3 months’ treatment (P<0.01), but there is no difference between conservative treatment and surgery in long-term(6 months -1 year after treatment),P> 0.05.After 3 months’ treatment,the result of conservative treatment,excellent 15,good 12,the rate of goodness is 27/30(90%),and the result of surgery,excellent 26,good 3,the rate of goodness is 29/30 (96.67%).After 6 months’ treatment,the result of conservative treatment, excellent 24,good 5,the rate of goodness is 29/30(96.67%),and the result of surgery,excellent 27,good 2,the rate of goodness is 29/30 (96.67%).After 1 year’s treatment,the result of conservative treatment,excellent 22,good 6,the rate of goodness is 29/30 (93.33%),and the result of surgery.excellent 26,good 3,the rate of goodness is 29/30 (96.67%).Conservative treatment or operation treatment has the Similar efficacy in the JOA score and the JOA improvement rate after 6 months or 1 year as well as 3 months’ treatment.(P>0.05) (2) Conservative treatment with YiqiZhuyu and Lishui Decoction and surgery both can improve the patients’ quality of life significantly (P< 0.01).Surgery has a better curative efficiency in short-term(3 months).In long-term(6 months～1 year later)) there is no difference between conservative treatment and surgery in the quality of the patients’ life in physical health(physical functioning. role physical health, bodily pain, general health perceptionsvitality).But in spiritual-mental health,surgery is better than conservative treatment in long term.Conclusion:(1) Conservative treatment with YiqiZhuyu and Lishui Decoction is one of the efficiency way to treat the ruptured lumbar disc herniation,and it can improve the patients’ quality of life significantly.(2) Although surgery has a better efficiency and higher quality of life in short-term treatment, there is no difference between conservative treatment and surgery in the improvement with symptoms, signs and daily activities,so we should make the operation indication strictly.Also we should make patients be more patience to the treatment,as well as make them cooperate with the treatment consistently.(3) We should pay more attention to the spiritual-psychological level of the conservative patients,and value communication, interactions, and other exposures with patients.Help them be more confidence in treatment,and quell fears.
