| Objective:The aim of this study is to analysis the influence on the corneal astigmatism and the stability of tear film after operation with two different kinds of incisions(2.2mm and 3.0mm) in patients with age-related cataract.Methods:The patient who conform to the requirements,according to the order of coming to the hospital, from March 2014 till June 2014 at the Second Affiliated Hospital of Dalian Medical University, who would undergo Phacoemulsification combined with intraocular lens implantation. 80 patients(80 eyes)who have age-related cataract were selected from our eye center. Patients were divided into 2 groups(40 eyes per group), group A has astigmatism with the rule,group B has astigmatism against the rule.Group A randomly divided into two groups,group A1,which has astigmatism with the rule choose 12 o’clock axial with 2.2mm corneal incision, group A2 which has astigmatism with the rule choose12 o’clock axial with 3.0mm corneal incision.Group B randomly divided into two groups,group B1 which has astigmatism against the rule choose 12 o’clock axial with 2.2mm corneal incision, group B2 which has astigmatism against the rule choose 12 o’clock axial with 3.0mm corneal incision.Check in the patient’s naked eye vision,best corrected eye visual acuity,astigmatism degree and the rupture time of tear film, among 10:00-12:00 at 1day before operation, at 1 day,1week,1 month and 3 months after operation. Combine with the date,compare A2 with A1,B2 with B1 to observe the inflience of the corneal astigmatism and compare A2 with A1,B2 with B1’s stability of tear film.Applying the statistical methods to analysis.Results: In A1ã€A2ã€B1ã€B2,the corneal astigmatism is the largest in the day after surgery, with the passage of time, the corneal astigmatism decreased gradually, and in 1month after surgery the astigmatism gradually become stable.There were no statistically significant difference observed in the comparison between A1 with A2,B1 with B2 in the patient’s astigmatism degree in 1day after operation(P>0.05);There were statistically significant difference observed in the comparison between A1 with A2 in the patient’s astigmatism degree in 1week,1month and 3months after operation(P<0.05);There were no statistically significant difference observed in the comparison between B1 with B2 in the patient’s astigmatism degree 1week,1month and 3months after operation(P>0.05); In A1ã€A2ã€B1ã€B2,the rupture time of tear film is the shortest in the day after surgery, with the passage of time, the the rupture time of tear film prolonged gradually, and in 1 month after surgery the the rupture time of tear film gradually become stable.There were statistically significant difference observed in the comparison between A1 with A2 in the patient’s rupture time of tear film in 1day,1week after operation(P<0.05);There were statistically significant difference observed in the comparison between B1 with B2 in the patient’s rupture time of tear film in 1day after operation(P<0.05);There were no statistically significant difference observed in the comparison between B1 with B2 in the patient’s rupture time of tear film in 1week after operation(P>0.05).Conclusion:( 1) With clear corneal incision Phacoemulsification combined with intraocular lens implantation for cataract patients who has astigmatism with the rule,choose 12 o’clock axial with 3.0mm clear corneal incision’s has more influence than2.2mm clear corneal incision after surgery.( 2) With clear corneal incision Phacoemulsification combined with intraocular lens implantation for cataract patients who has astigmatism against the rule,choose 12 o’clock axial with 3.0mm clear corneal incision and 2.2mm clear corneal incision’s has no more influence after surgery.(3)With clear corneal incision Phacoemulsification combined with intraocular lens implantation for cataract patients who has astigmatism with the rule,the 3.0mm clear corneal incision’s decrease much more than 2.2mm clear corneal incision on the stability of tear film in a certain period of time.(4)With clear corneal incision Phacoemulsification combined with intraocular lens implantation for cataract patients who has astigmatism against the rule,the 3.0mm clear corneal incision’s decrease much more than 2.2mm clear corneal incision on the stability of tear film at 1 day after operation. |
