Objective:Through the transplantation in alkali burn rabbit eyes with the different thickness keratoprosthesis (K-pro), and analysis of postoperative complications, the optimal thickness of K-pro was explored. Through the research of the PMMA (polymethylmethacrylate)-silicone K-pro implantation for the same level of alkali burned rabbit corneas by different operation modes, analysis the relationship between the operation and keratoprosthesis transplantation postoperative complication. Exploring the most suitable operation mode for the keratoprosthesis transplantation.Methods:It was would be made that the thirty rabbits’right eyes to establish alkali burn model with neovascular and stable corneal leucoma. Randomly selected eight right eyes were transplanted with the PMMA-silicone keratoprosthesis of the different thickness, including 0.1mm,0.35 mm and 0.5 mm. Postoperative complications were analyzed such as anterior proliferative membrane. In addition, the remaining 22 alkali burned rabbit eyes were randomly divided into A, B two groups,10 rats in group A,12 rats in group B. The thickness of 0.5mm keratoprosthesis implanted into two groups alkali burn rabbit eyes using two different operation methods. We observed the postoperative complications. Statistical analysis was performed between the two groups.Results:The alkali burn rabbit eyes with the thickness of 0.1mm keratoprosthesis had the former membrane after one month (3/3 ratio). One rabbit eye with the thickness of 0.35mm keratoprosthesis had the former membrane after two months (1/3 ratio). The former membrane does not appear in the rabbit eyes with the thickness of 0.5mm keratoprosthesis in three months (0/2 ratio). The time in group A rabbit eyes is range from 66 to 183 days, average 92.10 days; group B is located in 68 to 203 days, average 132.25 days. Seven eyes in A group rabbits have intraocular hypertension after operation. Only two eye intraocular pressure increased significantly after operation in B group rabbit eyes. Only one eye in group A showed the proliferative membrane around the cylinder lens of keratoprosthesis. All rabbit eyes of artificial cornea keep in the normal position more than three months have ultrasonic diagnosis B in three months after operation. Only in group A, one rabbit eye can see anterior vitreous opacity and a small amount of proliferation of the cable. The comparison between the two groups has statistically significant about the time in the normal position and high intraocular pressure.Conclusion:1. The K-pro with thickness of 0.5mm is more suitable for transplantation.2. The keratoprosthesis implantation is more feasible using the operation method of B group. Its time in the normal position is relatively long and postoperative complications are relatively few.
