Study Of POCD Incidence After Joint Replacement Surgery Due To The Use Of Sedatives In Perioperative

Purpose: This paper discusses the influence of the usage of sedatives on POCD incidence in Perioperative for Elderly patients with femoral neck fracture who do the artificial femoral head replacement in right time,so to provide a theoretical basis for further clinical application.Method: By collecting the cases data,144 cases in total, of the elderly patients with femoral neck fracture who did the arthoplasty during March.2013 to Dec.2014 in orthopedics department of the affiliated hospital of Shandong traditional Chinese medicine university.all the patients are beyond 70 years old,the average age is 81.90.Among that,male takes 52 cases,female 92 cases. The data including the sex, age, occupation, education level, underlying diseases preoperative, operative blood loss, operative time ect.Take the day before surgery,12 hours after surgery and 10 days post surgery as test basis for univariate analysis.Using the MMSE and MOCA to score the 3 basis and detect it by T test.Standard p<0.05 as significantly statistical difference.All the data are analyzed and finished by the SPSS17.0 Statistical SoftwareResults: After t test single factor analysis,we get results as follows:1. Comparing the scores of 10 days’ neuropsychological test between preoperative and postoperative for patients,p>0.05,has no statistical significance,which shows there is no big preoperative cognitive status difference before surgery.The results is comparable.2. Comparing the scores of 12 hours’ neuropsychological test within preoperative and postoperative for patients,p<0.05,has statistical significance,which shows the cognitive status of patient after stimulus like surgical trauma has distinct declination.3. Comparing the scores of 12 hours’ neuropsychological test between preoperative and postoperative,p<0.05,has statistical significance, Which shows the usage of sedatives in Perioperative can effectively minish the change degree of patients’ cognitive impairment,so to lower the POCD incidence4. Comparing the scores of neuropsychological test between the 12 hours and 10 days of post-surgery,within the group,P<0.05.has statistical significance which shows the cognitive status declination of the two group patients after the stimulus like operative trauma will be slowly improved in course of time.Conclusion:Through the observation of the elderly arthroplasty patents’ mental state,the two group Perioperative clinics use the alprazolam 0.8mg(experimental group)and the starch group with similar appearance(comparison group) to get the conclusion as follows:1.The usage of sedatives during perioperative can reduce the incidence of POCD within a certain range, can be an effective treatment for the prevention of incidence of postoperative cognitive dysfunction in Perioperative2. For the patients who troubled by the anxiety,fear and poor quality sleeping, the appropriate application of sedatives in Perioperative can release the stress and improve the sleep quality,in the mean time,reduce the incidence of postoperative POCD...
