Objective Using Color Doppler Ultrasound to observate the hemodynamics parameters information of inferior epigastric artery perforator arteries of healthy volunteers, to explore the application value of color Doppler ultrasound in preoperative localization and design of the deep inferior epigastric artery perforator flap transplantation.Methods According to the inclusion criteria and exclusion criteria, 150 healthy volunteers were included in the study, including male 126 cases, female 174 cases, age from 20 to 69 years(mean 41 years), according to the age groups were divided into 5 groups, 20 ~ 29 years old group, 30 ~ 39 years old group, 40 ~ 49 years old group, 50 ~ 59 years old group, 60 ~ 69 years old group, each group of 30 cases and 60 subjects blood vessels.The subjects in a supine position, expose the bilateral inguinal region, measure the length of inferior epigastric artery along its path(from a point to the first perforator distance L),count the number of perforator vessels, measure the diameter of D1(mm) in point of the external iliac of inferior epigastric artery, measure the diameter of the D2(mm) again at the level of the umbilical, and set the sample volume 1mmin the lumen central, adjust the probe angle to show the best pulse Doppler spectrum, measure the blood flow parameters, including peak systolic velocity(PSV), end diastolic velocity(EDV), pulsatility index(PI), resistance index(RI), the parameters above are measured for 3 times, and take the average value. The data are expressed as the mean ± SD. The parameters of the value are analysis statistically by the SPSS 17 for windows statistical software package, significant test between different age groups by analysis of variance.Results All 300 of the inferior epigastric artery can be clearly display by the Color Doppler ultrasound, the display rate is 100%, thrombosis, atherosclerosis plaque is not found, there is no blood vessels and abnormal phenomenon of walking. We compare the different ages of subjects, found that it is significant statistically in all age groups among the L, D1, D2, PSV, PI groups.1 Detection of inferior epigastric artery by Color Doppler ultrasound,283(92.66%) of inferior epigastric artery originate from the external iliac artery, 17(7.34%) from the femoral artery. The inferior epigastric artery issues, runs upwards into the sheath of rectus abdominis above the pubic symphysis(5.46±0.68)cm, move up to the umbilical between the rectus abdominis muscle and the posterior rectus sheath, forms abundant rami anastomoticus with superior epigastric artery at the level of last tendinous above the umbilical.2 The average length of inferior epigastric artery trunk is(7.83±0.55) cm,the average length of inferior epigastric artery is(13.35 ±1.94) cm.3 the average aperture at the startingpoint of arteriae epigastrica inferior is(2.27±0.35)cm, the average aperture of arteriae epigastrica inferior at the level of Umbilical is(0.76±0.24)cm.4 Both of the inferior epigastric artery send 3-8 perforators, if the caliber of the vessel≥0.5mm is useful, the number of vessels can be used is 1-3, the average(1.96± 0.91).Conclusions The study found that it is statistical significance among the inferior epigastric artery L, D1, D2, PSV, EDV, PI of the different age groups.It is suggested that it is real and effective to detect the path, diameter and Hemodynamic parameters of deep inferior epigastric artery perforator by Color Doppler ultrasound. Which can provide accurate,valueable for the preoperative design of the DIEP flap.Besides, the study found that the hemodynamic parameters of 60 ~ 69 years old group are different from other age groups, which is significant,while other groups do not show this phenomenon.This may relate to abdominal wall tissue ageing, muscle relaxation, vascular compliance decreasing, vascular wall elasticity variation of someone over 60.
