Objectives To observe the efficacy and safety of ropivacaine in cesarean section in patients with preeclampsia.Methods 84 cases with preeclampsia in Jilin Central Hospital from Oct 2013 to Aug 2014 were enrolled. All of them were singleton pregnancy and gestational age over 28 weeks. They were divided into two groups. The experimental group was given ropivacaine injection treatment, which the control group were given bupivacaine injection during cesarean section. All the patients were recorded including blood pressure, heart rate; and peripheral oxygen saturation as well before and post cesarean section. The occurrence of adverse reactions, complications, mortality rate; ICU occupancy rate, length of hospital stay were studied too. For the neonatal, Apgar score, NACS score as well as blood gas analysis from umbilical cord were studied. All the data were analysised in SPSS 17.0 software.Results There were no difference in baseline data, including age, gestational age, height, weight between two groups. There were no difference in heart rate, systolic blood pressure, diastolic blood pressure before operation between the two groups. There were no difference in complications including retinal damage, heart failure, cerebrovascular disease, complications related to indicators of acute kidney injury, HELLP syndrome, DIC, placental abruption, fetal distress. ASA grading for anesthesia preoperative showed no significant difference. The heart rate, systolic blood pressure, diastolic blood pressure, peripheral oxygen saturation during operation were different between the two groups. The heart rate, systolic pressure, diastolic pressure after the operation is better than that in control group. Incidence of complications such as hypotension, vomiting, pruritus and urinary retention were lower in the experimental group than those in the control group. There were difference for NRS pain scores at 4 hours after operation between two groups. There was less vomiting occurred in the experimental group than that in the control group. The average hospitalization time and ICU occupancy rate is lower in the experimental group than that in the control group. There was higher Apgar score at 1min and the 5min in the experimental group than that in the control group. The NACS scores in the experimental group was better than the control group. There were no difference in the neonatal umbilical cord blood gas analysis results of lactic acid, blood p H, PO2, PCO2, blood BE value between two groups.Conclusions Ropivacaine can effectively improve the patients’ blood pressure, heart rate, and can maintain the analgesic effect for a long time in patients with severe preeclampsia. There were less incidence of complications in bupivacaine group. Ropivacaine reduce the occurrence of fetal distress as well.
