Objective:1.To analysis application value of impulse oscillometry in chronic obstructive pulmonary disease。Methods:Impulse oscillmetory(IOS) and pulmonary function test(PFT) were performed in 140 subjects(including 98 patients with COPD and 42 normal subjects).All of them the m MRC and the AECOPD history were also required.Then the COPD patients were divided into 4 groups according to the diagnosis and treatment guildelines of COPD in China(2013 Revied).Results1.There was statistically significant difference between COPD group and normal group according to IOS measurements and PFT measurements.In the COPD group,resonant frequency(Fres),respiratory resistance at 5Hz(R5) and difference between respiratory resistance at 5Hz and 20Hz(R5-R20) were all correlated negatively with forced expiratory volume in one second total predicted value(FEV1%pred), forced vital capacity total predicted value(FVC%pred) and ratio between forced expiratory volume in one second and forced vital capacity(FEV1/FVC).Reactance at 5Hz(X5) was correlated positively with FEV1%pred,FVC%pred and FEV1/FVC.However,respiratory resistance at 20Hz(R20) was not related to any of these measurements.The strongest association were observed between FEV1%pred with X5.The strongest association were observed between FEV1/FVC with Fres.2.The sequence of diagnosis value of parameters measured by IOS for COPD is Fres(0.893)>X5(0.870)>R5(0.823)>R5-R20(0.762).3.Fres、X5:All groups had significant difference with each others(P<0.05),in addition to between group B with group C.R5 and R5-R20: There were significant difference between group D with other groups(P<0.05).R20:There were no singificant difference in all of groups(P>0.05).4.Fres,R5 and R5-R20 had negative correlation with combined COPD assessment(r=0.445,0.497,401).X5 has positive correlation with combined COPD assessment(r=-0.558).Conclusion1.Fres,R5,R5-R20 and X5 had significant relationship with PFT measurements.These can be used in diagnosing COPD.ALL of them Fres is the best measurements.2.Fres,R5,R5-R20 and X5 in COPD staging are showing the same degree of risk with the diagnosis and treatment guildelines of COPD in China(2013 Revied).These can be used to reflect the severity of COPD patients.
