Curative Effect Analysis Of CO₂ Laser Surgery In Treatment Of Early Glottic Carcinoma

Objective: To summarize clinical therapeutic effect of the CO2 laser surgery of early glottic carcinoma.Methods: a retrospective analysis of CO2 laser surgery of the clinical data of 43 patients with early glottis type of laryngeal cancer in 2008-2012 in fujian provincal hospital otolaryngology head and neck surgery.Patients aged 30-75 years old including male 38 cases and female 5 cases, among them Tis 5 cases,T1 a 22 cases,T1 b 7 cases, T2 cases.After postoperative follow-up of 3 years, analyzing the curative effect and the recurrence rate.Results: 43 patients were smoothly got CO2 laser surgery under the laryngoscope, all of them, without tracheotomy and there were no obvious complications.In the 5 cases of Tis, 4 under the resection of Ⅱ type, 1 under type Ⅲ excision, the recurrence rate is 0%(0/5), and 22 cases in T1 a, 2 underwent Ⅱ type resection, 13 cases under resection of Ⅲ type, 6 underwent resection Ⅳ type, 1 in type V excision and the recurrence rate is 4.55%(1/22).In 7 cases of T1 b,1 was in resection of Ⅲ type, 4 in Ⅳ resection type and 2 cases in resection of Ⅴ type, the recurrence rate is 14.3%(1/7). 9 cases of T2 were all performed Ⅴ excision, the recurrence rate is 22.2%(2/9).There was no statistically significant difference between the four groups(χ2 = 3.09, p > 3.09). In the 43 patients, infringement of former united reached 12 cases and the recurrence rate was 25.0%(3/12), before infringement of vocal cords jointly were 31 cases, the recurrence rate is 3.23%(1/31) the difference was statistically significant between the two groups(χ2 = 4.86, 0.01 < p < 0.05).Conclusion: CO2 laser surgery of early glottic carcinoma is safe and effective and it is the ideal solution for the treatment of early glottic carcinoma.
