| Objective:The treatment of anaphylactoid purpura in traditional Chinese medicine has the problem of non-standard syndrome differentiation and lacks of summary and research on compatibility of medicines. On the basis of collecting and processing the periodical literature of traditional Chinese medicine about anaphylactoid purpura, this study reviews and standardizes the syndrome differentiations of this disease, proposes a more practical way of syndrome differentiation and summarizes the rules of medicine application with the statistical method. It is hoped that this study can provide some guidance for clinical diagnosis and treatment of anaphylactoid purpura in traditional Chinese medicine.Methods:This study collected all the literatures from January 1982 to December 2012 about the syndrome differentiations and related indicators of anaphylactoid purpura and performed a statistical analysis on the differentiations, as well as all the symptoms, signs, prescriptions and related indicators that mentioned in the literatures by using data processing analysis ways like Golden Section to find the basis for syndrome differentiations and medicine application rules of anaphylactoid purpura.Results:This study made a research on the TCM syndrome types of anaphylactoid purpura by reviewing the literatures and drew a conclusion that there were six main types, including bleeding due to blood-heat, fire excess from yin deficiency, injury of collaterals due to wind-heat, blockage in the body due to damp-heat, obstruction of collaterals by blood stasis, Qi dysfunction in blood control, etc. After a comprehensive analysis on the symptoms of each syndrome type, as well as the tongue and the pulse, the core symptoms and secondary symptoms of the above six main types were confirmed. Through analyzing the drug-use frequency of one medicine, the drug-use frequency of main Chinese herbs and the commonly used formulas for the main syndrome types, the order of the frequency of common medicines and the main Chinese herbs in treating anaphylactoid purpura were confirmed. In the meantime, the commonly used Chinese herbs and the categories of common Chinese herbs and the shared Chinese herbs for each type were determined. In addition, the study defined the common classifications and rules of medicine application for anaphylactic purpura nephritis and made a comparison between two classifications and rules of medicine application by doing another research on renal damage accompanied by anaphylactoid purpura in the literatures.Conclusion:1. The clinical diagnose and treatment of anaphylactoid purpura in traditional Chinese medicine should be based on two types: blockage in the body due to damp-heat and obstruction of collaterals by blood stasis. 2. The study confirms the core symptoms and the pulse of main types of anaphylactoid purpura. The type of bleeding due to blood-heat is characterized by acute onset and purpura on the skin, accompanied by thirst, pyrexia, pharyngalgia, red tongue, yellow tongue coating and rapid pulse. The type of fire excess from yin deficiency is characterized by prunosus skin, long course and tending to recrudesce, accompanied by vexation, night sweat, dry stool, red tongue, little coating and rapid but thin pulse. The type of injury of collaterals due to wind-heat is characterized by bright-red purpura on the skin and pyrexia, accompanied by symptoms due to external factors, renal injury, arthrodynia, pharyngalgia, acute disease degree, red tongue, thin and yellow coating, superficial and rapid pulse. The type of blockage in the body due to damp-heat is characterized by purpura on the skin, accompanied by anorexia, nausea, abdominal pain, renal injury, hemafecia, red tongue, yellow and greasy coating, rapid and slippery pulse. The type of obstruction of collaterals by blood stasis is characterized by purpura on the skin and repeated illness, accompanied by renal injury, red tongue with ecchymosis, and wiry pulse. The type of Qi dysfunction in blood control is characterized by lavender purpura over the whole body, fatigue, slow disease degree and long course, accompanied by shortage of breath, renal injury, pale complexion, dizziness, pale tongue, thin and white coating, and thin and weak pulse. 3. The necessary herbs in treating each type of anaphylactoid purpura are as follows: heat-clearing drugs, relieving drugs, hemostatic drugs, promoting blood circulation and removing stasis drugs, and tonic drugs. 4. The core types of anaphylactoid purpura are: bleeding due to blood-heat, fire excess from yin deficiency, injury of collaterals due to wind-heat, blockage in the body due to damp-heat, obstruction of collaterals by blood stasis, and Qi dysfunction in blood control. The major types of anaphylactic purpura nephritis are: Qi dysfunction in blood control, injury of collaterals due to wind-heat, obstruction of collaterals by blood stasis and blockage in the body due to damp-heat, which are arranged from the highest drug-use frequency to the lowest. Compared both diseases, anaphylactic purpura nephritis has no types of bleeding due to blood-heat and fire excess from yin deficiency. 5. The major kinds of Chinese herbs in the treatment of anaphylactoid purpura are: heat-clearing drugs, tonic drugs, relieving drugs, hemostatic drugs, promoting blood circulation and removing stasis drugs; the major kinds of herbs for anaphylactic purpura nephritis are: heat-clearing drugs, tonic drugs, hemostatic drugs, promoting blood circulation and removing stasis drugs, clearing damp and promoting diuresis. Among them, heat-clearing drugs, promoting blood circulation and removing stasis drugs, hemostatic drugs and tonic drugs are commonly used in both diseases. |
