| Background and Objective: Heart failure is the end stage of various heart disease.It is a common clinical disease, which seriously harms people’s health and longevity. Presently, there is no specific method for the diagnose, treatment and prognosis of heart failure. The treatment and prognosis of different patients with different cardiac function is different, so the accurate evaluation of cardiac function in patients with heart failure is very important for clinical diagnosis and treatment and prognosis judgement. Troponin is the main regulating muscle protein, which is composed of three subunits.It is a regulatory protein of voluntary muscle contraction and one of the cardiac specific protein.It is the markers of myocardial injury with high sensitivity and high specificity. This study is based on the relationship between heart failure and troponin I, hoping to provide assessment of clinical diagnosis, treatment and prognosis of heart failure.Objects:100 cases of healthy people and 100 patients with congestive heart failure hospitalized in my Department during April 2013-March 2014 were selected.Methods:The heart function in patients with congestive heart failure is divided into II, III and IV level acording to NYHA classification, healthy cases as control group.Cardiac troponin I and cardiac ultrasound were measured whithin 24 hours of admission,comparing the differences of cardiac function cardiac troponin I and observing the correlation between cardiac troponin I and EF values.1.Patients with congestive heart failure is divided into non-coronary heart disease group and coronary heart disease group, comparing the differences of Troponin I in patient of different etiology.2.Patients with congestive heart failure with elevated cardiac troponin I were divided into two groups,one with cardiac troponin I recovered normal, the other remaining high cardiac troponin I level.tracking-down the rehospitalization rates,death rates, number of new-onset atrial fibrillation, and new number of complete left bundle branch block of the patients, judging the assessment and prognosis of cardiac troponin I in patients with congestive heart failure.Results:Cardiac troponin I and cardiac function, LVEF value was negatively correlated, the worse of heart function(p<0.001), cardiac troponin increased more significantly(p<005), the lower the LVEF value(p<005).Elevated cardiac troponin I in patients with heart failure has nothing to do with causes(>005).Compared with the group whose cardiac troponin I recovered normal, the rehospitalization rate, the rate of sudden death of patients, the number of new onset atrial fibrillation, new onset of complete left bundle branch block number in patients remaining high cardiac troponin I level after treatment were increased, the difference was statistically significant(p<005). |
